针灸联合西药治疗脾肾阳虚型系统性硬皮病的疗效及对血清免疫、胶原代谢相关因子的影响

摘　　要：目的:观察针灸联合西药治疗脾肾阳虚型系统性硬皮病的疗效及对血清免疫、胶原代谢相关因子的影响。方法:将80例系统性硬皮病脾肾阳虚证患者随机数字法分为对照组与观察组,每组各40例。对照组单纯给予西药治疗,观察组在对照组基础上给予针灸治疗,观察两组临床疗效及治疗前后主要中医症状积分的变化情况,并检测两组免疫与胶原代谢相关生化指标水平。结果:两组治疗后的畏寒肢冷、神疲乏力、腰酸无力、舌苔薄白症状积分均有明显降低(P<0.05),且观察组的各项症状积分明显低于对照组(P<0.05)。观察组治疗总有效率明显高于对照组(P<0.05)。两组治疗后的CD3+、CD4+、CD4+/CD8+指标水平均较治疗前提高,CD8+指标水平较治疗前降低,差异均有统计学意义(P<0.05);且观察组的变化幅度大于对照组(P<0.05)。两组治疗后的CTGF、TMP-1、TMP-2水平较治疗前降低,MMP-1较治疗前升高,差异均有统计学意义(P<0.05);且观察组的变化幅度大于对照组(P<0.05)。结论:针灸联合西药治疗脾肾阳虚型系统性硬皮病疗效确切,可有效改善脾肾阳虚的症候,改善失衡的免疫系统和调节异常的胶质代谢,促进皮肤组织的恢复。Objective:To observe the curative effect of acupuncture and moxibustion combined with western medication in the treatment of systemic sclerosis of the deficient pattern of spleen-yang and kidney-yang,and to investigate its influence to sero-immune and collagen metabolic related factors.Methods:80 patients with systemic scleroderma were randomly divided into the control group(n=40)and the observation group(n=40).The control group was treated with western medicine alone;on which basis,the observation group was also given the treatment of acupuncture and moxibustion.The clinical curative effect and the changes of the main TCM symptoms were observed before and after the treatment,and the biochemical indexes related to immune and collagen metabolism were detected.Results:The symptom scores of the two groups were significantly decreased after the treatment in the two groups(P<0.05);of which the observation group was obviously lower than the control group(P<0.05).The total effective rate of the observation group was significantly higher than that of the control group(P<0.05).The levels of CD3+,CD4+,CD4+/CD8+were significantly higher after the treatment than those before the treatment in the two groups(P<0.05),and CD8+level was reduced after the treatment(P<0.05).The levels of CTGF,TMP-1 and TMP-2 were lower after the treatment than those before the treatment in the two groups(P<0.05),and MMP-1 level was higher after the treatment(P<0.05).All above changes were more significant in the observation group compared to those in the control group(P<0.05).Conclusion:The therapy of acupuncture and moxibustion combined with western medication has obvious curative effect on systemic sclerosis,which can effectively improve the deficiency symptoms of spleen-yang and kidney-yang,improve the imbalance of immune system and regulate abnormal collagen metabolism,and promote the recovery of skin tissue.
