血浆白蛋白(Alb)及血红蛋白(Hb)水平与重型颅脑损伤患者预后的相关性

摘　　要：目的探讨重型颅脑损伤(STBI)患者血浆白蛋白(Alb)及血红蛋白(Hb)水平与预后的相关性。方法方便选取2016年5月—2017年4月该院ICU收治的64例STBI患者为研究对象,依治疗方式不同将入选病例分为两组。对照组32例,患者入院后立即给予及时正确地急救处理、依指征行脑外科手术及非手术治疗;实验组32例,患者在对照组治疗基础上输注人血白蛋白和悬浮红细胞以维持正常Alb及Hb水平。记录两组STBI患者治疗期间血浆Alb水平、Hb水平及格拉斯哥昏迷评分(GCS),对比观察Alb及Hb水平与STBI患者预后的相关性。结果实验组生存率90.6%,死亡率9.4%,Alb(34.6±2.8)g/L,Hb(99.3±6.4)g/L,各项指标均优于对照组(P<0.05),Alb<30 g/L、Hb<90 g/L是STBI患者死亡危险因素。结论血浆白蛋白及血红蛋白水平与重型颅脑损伤患者预后存在密切关联,维持Alb≥30 g/L、Hb≥90 g/L对STBI患者预后具有重要意义。Objective This paper tries to investigate the correlation between serum albumin(Alb)and hemoglobin(HB)level in patients with severe craniocerebral injury(STBI).Methods 64 cases of STBI patients in the hospital from May 2016 to March 2017 were convenient selected and were divided into two groups according to the treatment methods.The control group of 32 cases,patients were given prompt and proper first aid treatment immediately after admission,according to the indications of brain surgery and non-surgical treatment;experimental group of 32 patients,based on the control group added the infusion of human serum albumin and suspended red blood cells to maintain normal Alb and Hb Level.The levels of Alb,Hb and Glasgow Coma Scale(GCS)were recorded during treatment in two groups of STBI patients.The correlation between Alb and Hb levels and the prognosis of STBI patients was compared.Results The survival rate of experimental group was 90.6%,mortality rate was 9.4%,Alb(34.6±2.8)g/L,Hb(99.3±6.4)g/L,significantly better than the control group(P<0.05),Alb<30 g/L,Hb<90 g/L as the death risk factors of STBI.Conclusion The plasma albumin and hemoglobin level are closely correlated with the prognosis of severe craniocerebral injury,and to maintain Alb≥30 g/L and HB≥90 g/L is important to the prognosis of patients.
