腹腔镜与开腹胰头十二指肠切除术的疗效及经济效益对比

摘　　要：目的从住院总费用等经济效益角度探讨腹腔镜胰头十二指肠切除术作为常规手术的可能性。方法回顾分析我院收治的41例行胰头十二指肠切除术患者的临床资料,其中20例行腹腔镜手术(腹腔镜组),21例行开腹手术(开腹组)。比较两组患者手术相关指标及经济效益情况。结果腹腔镜组排气时间和下床活动时间明显早于开腹组(P<0.05);两组手术时间、术中出血及术后住院时间比较,无明显差异(P>0.05)。腹腔镜组住院总费用及耗材费用显著高于开腹组(P<0.05);两组西药费用及手术费用比较,差异无统计学意义(P>0.05)。结论腹腔镜胰头十二指肠切除术较开腹胰头十二指肠切除术具有术后排气快、下地活动时间早等优点,且术后用药费用、手术费用与传统开腹手术基本持平,但住院总费用、耗材应用较开腹有所升高。将腹腔镜胰腺十二指肠切除术作为常规方式甚至标准方式具有可行性。Objective To discuss the possibility of laparoscopic pancreaticoduodectomy as a routine operation from the perspective of economic costs such as total hospitalization expenses.Methods The clinical data of patients with pancreatic diseases underwent pancreaticoduodenectomy in our hospital were analyzed retrospectively.A total of 20 cases underwent laparoscopic pancreaticoduodenectomy(laparoscopic group),and 21 cases underwent laparotomy pancreaticoduodenectomy(laparotomy group).The operation related indexes and economic costs of the two groups were compared.Results The time of exhaust and the time of ground movement in the laparoscopic group were significantly earlier than those in the laparotomy group(P<0.05).There were no significant differences in operative time,intraoperative bleeding and postoperative hospitalization time between the two groups(P>0.05).The total cost of hospitalization and cost of consumables in the laparoscopy group were significantly higher than those in the laparotomy group(P<0.05).The cost of western medicine and the cost of operation had no statistically significant differences between the two groups(P>0.05).Conclusion The laparoscopy pancreaticoduodectomy has the advantages of faster postoperative exhaust and lower time of activity on the ground than the laparotomy pancreaticoduodectomy,and postoperative medication cost and operation cost are same as those of traditional laparotomy surgery,but the total cost and the consumption of materials are higher than those in laparotomy.Laparoscopic pancreaticoduodectomy is feasible as a routine or even standard method.
