重组人源胶原蛋白凝胶联合创面磨痂术在深Ⅱ度烧伤患者中的应用

摘　　要：目的:探讨重组人源胶原蛋白凝胶联合创面磨痂术在深Ⅱ度烧伤患者中的应用效果。方法:将本院收治的86例深Ⅱ度烧伤患者随机分为研究组(43例)和对照组(43例),研究组给予重组人源胶原蛋白凝胶+创面磨痂术治疗,对照组给予凡士林油纱布+创面磨痂术治疗,比较两组术后创面愈合率、创面积液程度、黏连程度、疼痛评分等情况。结果:两组术后9d、27d创面愈合率比较,差异无统计学意义(P>0.05);研究组术后15d、21d创面愈合率明显高于对照组(P<0.05)。研究组创面愈合时间、疼痛评分、术后3d创面积液程度和黏连程度均显著优于对照组(P<0.05)。研究组术后6d创面分泌物细菌阳性率明显低于对照组(P<0.05)。研究组术后15d全身炎症反应综合征(SIRS)发生率显著低于对照组(P<0.05);术后3个月的创面瘢痕生长评分亦显著低于对照组(P<0.05)。两组治疗期间均未出现局部或全身过敏、肝肾功能异常等不良反应。结论:重组人源胶原蛋白凝胶联合创面磨痂术可有效促进深Ⅱ度烧伤患者伤口愈合,提高愈合质量,减轻患者痛苦,减少创面分泌物和创面瘢痕的生长。Objective:To investigate the effects of recombinant human collagen gel combined with wound crusting on deep second degree burns.Methods:86 cases of patients with deep second degree burn admitted to our hospital were randomly divided into the study group(n=43)and the control group(n=43).The patients in the study group were given the recombinant human collagen gel and wound crusting,and those in the control group were given the Vaseline oil gauze and wound crusting.The wound healing rate,wound fluid level,adhesion degree and pain score were compared between the two groups.Results:There was no significant difference in the wound healing rate between the two groups on day 9 and day 27 after operation(P>0.05).The wound healing rate in the study group on day 15 and day 21 after operation was signiti-cantly higher than that in the control group(P〈0.05),and the wound healing time,pain score,postoper-ative wound fluid level and adhesion degree in the study group were significantly better than those in the control group(P〈0.05).The positive rate of bacterial wound secretions in the study group on day 6 after operation was significantly lower than that in the control group(P〈0.05).The incidence of systemic in-flammatory response syndrome(SIRS)on postoperative day 1 5 in the study group was lower than that in the control group(P〈0.05).At 3 months after operation,the wound scars growth score in the study group was lower than that in the control group(P〈0.05).No local or systemic allergy and adverse reac-tions were noted in both groups.Conclusion:Recombinant human collagen gel combined with wound crus-ting could effectively accelerate the wound healing,improve healing quality,relieve pain and reduce the wound scars growth in patients with deep second degree burns.
