慢性阻塞性肺疾病患者外周血Th22及IL-22水平变化及其与肺功能的相关性研究

摘　　要：目的:探讨慢性阻塞性肺疾病(COPD)患者外周血辅助性T细胞22(Th22)及血清中白细胞介素(IL)-22水平及与肺功能的相关性。方法:选取COPD急性加重期22例(COPD急性加重期组)和COPD稳定期20例(COPD稳定期组),以同期本院健康查体者15名作为对照组。采用便携式肺功能仪测量两组呼气流量峰值(PEF%)、第1秒用力呼气量(FEV1%)、75%肺活量时用力呼气量占正常预计值百分比(V75%)、50%肺活量时用力呼气量占正常预计值百分比(V50%)、25%肺活量时用力呼气量占正常预计值百分比(V25%)。流式细胞仪测定外周血Th22细胞数量。酶联免疫吸附试验(ELISA)测定3组血清中IL-22水平。分析COPD患者外周血Th22及血清中IL-22与肺功能的相关性。结果:与对照组比较,COPD稳定期组和COPD急性加重期组患者肺功能指标PEF%、FEV1%、V75%、V50%、V25%均明显降低(P<0.01)。与COPD稳定期组比较,COPD急性加重期组患者以上肺功能指标下降更明显(P<0.01)。与对照组比较,COPD稳定期组和COPD急性加重期组患者外周血Th22及血清中IL-22水平均明显升高(P<0.01)。与COPD稳定期组比较,COPD急性加重期组患者外周血Th22及血清中IL-22水平升高更明显(P<0.01)。经Spearman相关性分析,COPD稳定期和急性加重期患者外周血Th22及血清中IL-22水平与肺功能指标PEF%、FEV1%、V75%、V50%、V25%呈负相关关系(P<0.01)。结论:COPD患者外周血Th22及血清中IL-22变化可作为衡量COPD患者病情严重程度的重要指标,且与肺功能呈负相关关系。Objective:To investigate the correlation between peripheral blood T helper 22 cells(Th22)and interleukin-22(IL-22)levels and lung function in chronic obstructive pulmonary disease(COPD)patients.Methods:22 patients with acute COPD and 20 patients with stable COPD were selected.15 healthy subjects were selected as a control group.Peak expiratory flow(PEF%),forced expiratory volume tn one second(FEV1%),and the ratio of forced expiratory volume to expected volume at 75%of vital capacity(V75%),50%of vital capacity(V50%),and 25%of vital capacity(V25%)were recorded.The number of Th22 cells tn peripheral blood was measured by flow cytometry.The serum level of IL-22 was detected by enzyme linked immunosorbent assay(ELISA).The correlation between Th22 and IL-22 levels and lung function was analyzed.Results:PEF%,FEV1%,V75%,V50%and V25%tn COPD patients were obviously de-creased compared to the controls(P<0.01),and were lower in acute COPD patients than those in stable COPD patients(P<0.01).Meanwhile,the levels of Th22 and IL-22 in COPD patients were notably in-creased compared to the controls(P<0.01),and were higher\n acute COPD patients than those in stable COPD patients(P<0.01).Spearman correlation analysis revealed that peripheral blood Th22 and IL-22 levels were negatively associated with the lung function of COPD patients.Conclusion:Peripheral blood Th22 and IL-22 levels could be important makers for evaluating the severity of COPD.There was negative correlation between Th22 and IL-22 levels and lung function of COPD patients.
