摘 要:目的:调查青海省西宁、玉树两地居民维生素D缺乏情况及代谢综合征(MS)发病情况,并探讨两者之间的相关性。方法:采用多级分层整群抽样法,选取2014年1月至2017年1月在本院健康体检者926例作为研究对象,收集所有对象身高、体重等一般资料,测定血清空腹血糖(FBG)、甘油三酯(TG)、总胆固醇(TC)、低密度脂蛋白胆固醇(LDL-C)、高密度脂蛋白胆固醇(HDL-C)及血清25-羟维生素D[25(OH)D]水平,分析维生素D缺乏与MS相关指标的关系。结果:926例研究对象中,维生素D缺乏154例(16.63%),MS 123例(13.28%),维生素D水平及MS发生率在西宁、玉树两地的分布均无明显差异(P>0.05);男性MS发生率高于女性(P<0.05),回族MS发生率高于汉族和藏族(P<0.05),且MS组和维生素D缺乏组平均年龄均高于非MS组或维生素充足或不足组(P<0.05)。MS组维生素D缺乏率明显高于非MS组(P<0.05)。MS患者中不同维生素D水平组间体重指数(Ibm)、FBG、TG、TC、LDL-C、HDL-C水平比较,差异均有统计学意义(P<0.05)。Logistic回归模型分析显示:性别、Ibm、腰围(WC)、血清25(OH)D、FBG、TG、HDL-C是发生MS的影响因素(P<0.05)。结论:西宁、玉树地区均具存在维生素D缺乏现象,且维生素D缺乏与MS相关代谢指标性别、BMI、WC、FBG、TG、HDL-C具有相关性。Objective:To investigate Vitamin D deficiency and metabolic syndrome(MS)in Xining and Yushu of Qinghai province.Methods:926 subjects underwent physical examination in our hospital from Jan-uary 2014 to January 2017 were selected.Height,weight and other general data were collected.Serum fasting blood glucose(FBG),triglyceride(TG),total cholesterol(TC),low density lipoprotein cholesterol(LDL-C),high density lipoprotein cholesterol(HDL-C)and serum 25-hydroxyvitamin D[25(()H)D]levels were detected.The relationship between vitamin D deficiency and MS related indexes was analyzed.Results:Of 926 subjects,there was 154 cases(16.63%)with vitamin D deficiency and 123 cases(13.28%)MS.There was no significant difference in the distribution of vitamin D level and MS incidence between Xining and Yushu(P>0.05).But the incidence of MS in male subjects was higher than that in female ones(P〈0.05),and was higher in Hui nationality than that in Han and Tibetan ethnic groups(P〈0.05).The average age in MS group and vitamin D deficiency group was higher than that in non-MS group or vitamin sufficient or in-sufficient group(P〈0.05).The body mass index,FBG,TG,TC,LDL-C and HDL-C among MS patients with different vitamin D level were statistically significant〈0.05).The Logistic regression model showed that sex,body mass index?waist circumference,and serum 25(OH)D,FBG,TG and DL-C levels were the factors influencing the occurrence of MS(P〈0.05).Conclusion:There is vitamin D deficiency in Yushu area of Xining province,and vitamin D deficiency is correlated with MS related metabolic indexes,such as gender,body mass index,waist circumference,FBG,TGand HDL-C.
