艾滋病HAART治疗生化计量指标异常率分析

摘　　要：目的探讨HIV感染患者HAART治疗前及治疗后生化指标异常率变化特征。方法选择HIV患者300例,在HAART治疗前及过程中连续监测,每年分别对各组艾滋病患者在肝功能、肾功能、血脂代谢、胰腺功能、血糖、乳酸等生化计量指标异常率的变化进行研究。结果在艾滋病患者未治疗组中各生化指标已有患者存在异常,其中以ATL、AST、TBA、GGT异常为主,异常率分别为9.0%、9.9%、10.3%、9.3%;GLU异常率为最低1.7%。与未治疗组进行对比,经HAART治疗1年后,TBA、GGT、TG、TC、LAC、GLU等异常率已出现增高(P<0.05),ALT、AST、UREA等治疗后第二年则开始出现增高(P<0.05),Cr在治疗至第3年才出现增高(P<0.05),AMS治疗前后异常率无显著增高。结论 HIV患者在HAART及病毒的共同影响下,较大比例患者出现代谢综合征。扩大监测各项生化计量指标异常率的监测,对发现可能出现的并发症类型,以便更早的进行针对性干预治疗具有重要意义。Objective To investigate the characteristics and changing of the biochemical measurement abnormal rate in patients who accepted highly active antiretroviral therapy(HAART).Methods Choose 300 cases of outpatients in our hospital whom accepted HAART from October 2013 to October 2016,Constant monitor the biochemical measurement abnormal rate of case’s lipid metabolism,bile metabolism,liver function,blood glucose metabolism,kidney function and lactic acid metabolism,then analyze the causes.Results Before HAART treatment,there had biochemical measurement abnormal found in part of HIV patients,among them,ATL(9.0%),AST(9.9%),TBA(10.3%),GGT(9.0%)have the higher abnormal rate and GLU(1.7%)has the lowest abnormal rate.Contrast with untreated group,after one year of HAART therapy,the abnormal rate of TBA,GGT,TG,TC,LAC,GLU already have significantly change(P<0.05);after two years of HAART therapy,the abnormal rate of ALT,AST,UREA have significantly change(P<0.05);Cr has significantly change in the third year of HAART therapy(P<0.05).The abnormal rate of AMS has no significant increase during three years of HAART.Conclusion Influencing by HAART and HIV virus,a large proportion of HIV patients occurs metabolic syndrome.Expanding monitor range of various biochemical measurement indexes abnormal rate is meaningful,especially in finding possible complication,and then we can fix it in time.
