西城某社区人群高尿酸血症危险因素的巢式病例对照研究

摘  要:目的探讨西城某社区人群高尿酸血症的危险因素,为预防高尿酸血症及其相关疾病的发生提供理论依据。方法采用便利抽样方法选取2015年1—12月西城某社区调查发现的1 200名血尿酸正常人群作为研究队列人群,进行为期2年的观察。采用巢式病例对照研究方法,以发现尿酸升高者作为此次病例研究的病例组,以1:2的比例为每例病例匹配两位对照,其中病例组选取130例,对照组选取260例,采用自行设计的调查问卷,进行包括个人基本资料、生活行为习惯、生活方式、疾病史等调查,同时进行体格检查及相关血液生化指标的检测,分析高尿酸血症的危险因素。结果 1 200名社区人群中,高尿酸血症130例,占10.83%,其中男性50例,占4.17%,女性80例,占6.67%,进行Logistic多因素分析,显示甘油三酯水平(OR=1.947,CI为1.368~2.77)、尿素氮水平(OR=1.237,CI为1.097~1.394)、海鲜摄入(OR=1.512,CI为1.051~2.177)、腰围(OR=1.022,CI为1.001~1.043)是高尿酸血症的危险因素,差异有统计学意义(P<0.05)。结论应警惕高尿酸血症的危险因素,提高社区对于高尿酸血症的综合管理能力。Objective To explore the risk factors of hyperuricemia in a community in Xicheng district,to provide theoretical basis for preventing hyperuricemia and its related diseases.Methods Using the convenience sampling method,1 200 patients with normal blood uric acid were selected as the study cohort in a community survey in Xicheng district from January to December 2015,a two-year observation.A nested case-control method was used to detect increased uric acid as the case group for this case study.Two controls were matched for each case from the cohort at 1:2.Among them,130 cases were selected in case group and 260 cases were selected in the control group.A self-designed questionnaire was used to conduct research including basic personal data,lifestyle habits,lifestyle,disease history and so on.At the same time,physical examination and related blood biochemical indexes were examined to analyze the risk factors for hyperuricemia.Results Among 1 200 community residents,130 cases had hyperuricemia,accounting for 10.83%.50 cases of them were male,accounting for 4.17%.80 cases were female,accounting for 6.67%.Logistic multi factor analysis showed that triglyceride levels(OR=1.947,CI,1.368~2.77),blood urea nitrogen(OR=1.237,CI,1.097~1.394),seafood intake(OR=1.512,CI,1.051 to 2.177),waist circumference(OR=1.022,CI,1.001~1.043)were risk factors of hyperuricemia,the difference was statistically significant(P<0.05).Conclusion The risk factors of hyperuricemia should be vigilant to improve the comprehensive management ability of the community for hyperuricemia.
