摘 要:目的研究幽门螺杆菌(Helicobacter pylori,H.pylori)毒力基因vac A的分布,分析H.pylori主要毒力基因vac A的多态性与胃十二指肠疾病关系。方法将胃黏膜标本培养分离H.pylori菌株提取DNA,并运用聚合酶链式反应(polymerase chain reaction,PCR)对毒力基因vac A及其亚型进行检测分析,并对实验数据进行统计学处理,分析H.pylori的毒力基因型vac A与胃肠疾病之间是否存在相关性。结果分离H.pylori 69株,标本依据病理诊断结果分为3组:消化性溃疡组(peptic ulcer disease,PUD)29株、非溃疡性消化不良组(non-ulcer dyspepsia,NUD)29株和胃癌组(gastric cancer,GC)11株。vac A基因s1的检出率为78.3%,s2为21.7%,mixs1为14.5%。m区m1和m2检出率分别为46.4%和53.6%。i区i1和i2的检出率分别为71.0%和29.0%。vac A基因亚型组合以s1m1i1(40.6%)和s1m2i1(34.8%)为主。结论本地区分离的H.pylori vac A s1m1i1基因型引起的疾病比例较高,是胃肠疾病的优势基因型,且vac A基因亚型组合s1cm2i2在各组疾病中的差异有统计学意义。而未发现其他基因型或基因组合与胃肠道疾病的关系。Objective To investigate the distribution of Helicobacter pylori(H.pylori)virulence genotypes vacA in gastrointestinal diseases and to analyze the correlation between the polymorphism of H.pylori virulence genotypes vacA and gastroduodenal diseases.Methods DNA was extracted from H.pylori strain by gastric mucosal culture,and the virulence genotypes vacA were analyzed by polymerase chain reaction(PCR).The experimental data were statistically analyzed by SPSS,version 19.0.Results Sixty-nine strains of H.pylori were isolated and divided into 3 groups:29 strains of peptic ulcer disease(PUD)group,29 strains of non-ulcer dyspepsia(NUD)group and 11 strains of gastric cancer(GC)group.The vacA genotype s1 was 78.3%,s2 was 21.7%and mixs1 was 14.5%.The m1 and m2 of m-region were 46.4%and 53.6%,respectively.The vacA i1 and i2 were 71.0%and 29.0%,respectively.The most prevalence vacA combination genotpyes were s1m1i1(40.6%)and s1m2i1(34.8%).Conclusion The vacA genotype s1m1i1 is the dominant genotype of gastrointestinal disease in our areas.The vacA s1cm2i2 has significant relationship with gastrointestinal diseases.No significant correlation is found between the other single or combination genotypes and gastrointestinal diseases.
