应用六西格玛评价河北省17家三甲医院凝血项目的检验性能

摘　　要：目的应用六西格玛(6σ)评价参加河北省室内质控室间比对的17家三甲医院的凝血实验项目数据的检验质量现状。方法收集该省17家三甲医院室内质控室间比对凝血实验项目室内质控数据,项目包括血浆凝血酶原时间(PT)、活化部分凝血活酶时间(APTT)、纤维蛋白原(FIB),统计各实验室室内质控均值()、变异系数(CV)、偏移(bias),西格玛(σ)度量和质量目标指数(QGI)。结果 PT、APTT、FIB大于3σ的性能优良实验室分别占58.82%、41.18%、52.94%;PT、APTT、FIB为2σ～3σ的中等质量实验室分别占17.65%、58.82%、35.29%;PT、APTT、FIB小于2σ性能较差的实验室分别占23.53%、0.00%、11.76%。需优先改进PT、APTT、FIB精密度的实验室占91.12%、82.35%、88.24%。结论参加比对的17家三甲医院凝血实验项目检验性能有待提高。6σ评价方法统一而简单,用于室内质控室间比对更加简便直观。Objective To assess the performance of the coagulation test performance of 17 laboratories of the major hospitals in Hebei province by six sigma,which the joint the internal laboratory comparison on quality control in Hebei province,so as to find problem,take measures and promote quality improvement.Methods Internal quality control data of 17 laboratories were collected,and the test items included prothrombin time(PT),activated partial thromboplastin time(APTT)and fibrinogen(FIB).Every mean value(X),CV,bias,sigma value(σ)and quality target index(QGI)were calculated.Results Laboratories ofσvalues of PT,APTT and FIB which were higher than 3σaccounted for 58.82%,41.18%and 52.94%.Laboratories ofσvalues of PT,APTT and FIB which were belong to 2σ-3σaccounted for 17.65%,58.82%and 35.29%.And laboratories ofσvalues of PT,APTT and FIB which were lower than 2σaccounted for 23.53%,0.00%and 11.76%,respectively.Laboratories which needed to improve precision firstly accounted for 91.12%,82.35%and 88.24%,respectively.Conclusion The coagulation testing performance of laboratories internal laboratory comparison in Hebei province needs to be improved.The six sigma method is an efficient way to control quality,and is helpful to continuous improvement.
