摘 要:目的:探究解脲支原体(Ureaplasma urealyticum,UU)和沙眼衣原体(Chlamydia trachomatis,CT)生殖道感染与不孕不育的相关性。方法:选取荆州市第一人民医院生殖中心2015年1月至2016年12月诊治的不孕不育患者482例,作为不孕不育组(其中夫妻91对);另随机抽取同时期健康已育体检者842例,作为对照组。用荧光定量PCR仪器检测患者生殖道分泌物标本中解脲支原体和沙眼衣原体DNA,采用SPSS软件进行统计分析。结果:UU感染率在不孕不育组与对照组分别为61.4%和40.9%(χ~2=51.865,P<0.05);CT感染率在不孕不育组与对照组分别为5.4%和2.3%(χ~2=9.191,P<0.05),解脲支原体和沙眼衣原体感染率在不孕不育患者中均较高。不孕不育组91对夫妻中丈夫与妻子的UU感染率分别为15.4%和48.4%,夫妻间UU感染并无相关性(r=0.075,P>0.05);91对夫妻中丈夫与妻子的CT感染率分别为9.9%和11.0%,夫妻间CT感染具有相关性(r=0.707,P<0.05)。结论:生殖道UU与CT感染与不孕不育存在一定相关性,监测UU与CT感染有助于不孕不育的病因诊断,并为不孕不育患者治疗提供指导依据。Objectives:To investigate the relationship between Ureaplasma urealyticum (UU)andChlamydia trachomatis (CT)genital tract infection and infertility.Methods:482 infertile patientsin the reproductive center of Jinzhou First People’s Hospital from January 2015 to December 2016 were selected as infertility group (91 pairs of couples),and another 842 cases of healthy physicalexamination of the same period randomly were selected into control group.Fluorescence quantitativePCR was used to detect Ureaplasma urealyticum and Chlamydia trachomatis DNA in patient’s genitaltract secretion samples,and SPSS software was used for statistical analysis.Results:Infection rate of UUin infertility group and control group were 61.4% and 40.9% (χ~2=51.865,P<0.05)respectively,and CTinfection rate in infertility group and control group were 5.4% and 2.3% (χ~2=9.191,P <0.05).Compared to healthy control group,infertile patients had higher infection rate of Ureaplasmaurealyticum and Chlamydia trachomatis.The UU infection rate of husband and wife in 91couples ofinfertile group were 15.4% and 48.4% respectively,and there was no correlation between the UUinfection in the husband and wife(r=0.075,P>0.05).The CT infection of husband and wife in 91 couples were 9.9% and 11% respectively,and the CT infection between husband and wife werecorrelated(r=0.707,P<0.05) .Conclusion:There is a certain correlation between UU and CT infection of reproductive tract and infertility,therefore,monitoring UU and CT infection couldcontribut to the diagnosis of infertility and provides guidance for the treatment of infertile patients.
