社会网络对心血管疾病患者医疗服务利用的影响研究

摘　　要：目的探究社会网络与心血管疾病患者医疗服务利用状态之间的关系。方法采用多层随机抽样方法,对重庆市11个社区服务中心登记管理的635名心血管疾病患者进行面对面问卷调查。基于社会网络的定位法,对网络顶端、网络差异、网络距离3个维度进行测量。应用多元Logistic回归模型分析社会网络对心血管疾病患者医疗卫生服务利用状态产生的影响。结果被调查者中,接受正规医疗卫生服务的比例为26.9%。在控制人口学特征、经济社会特征以及健康状态之后,网络顶端和网络异质性能有效提高心血管疾病患者利用正规医疗卫生服务的可能性,其OR值分别为1.14(95%CI:1.01~1.39)和1.34(95%CI:1.04~1.71),而网络距离的影响差异无统计学意义。结论社会网络对心血管疾病患者是否利用正规医疗卫生服务有着显著影响,因此,需要教育或鼓励患者积极参与社会交往,注重提高交往的异质性和层次性。Objective To examine the association between social network and the use of medical care service among the patients with cardiovascular disease.Methods Using the multistage sampling design,634 respondents were randomly selected from 11 Community Service Centers of Chongqing.The questionnaires were administered face to face.Based on the Position Generator of social network,we measured the following 3 dimensions:network reach,network diversity and network range.The multivariate Logistic regression model was used to analyze the effect of social network on use of medical care service.Results The rate of using medical care service was 26.9%.Controlling for the socio-demographic,socioeconomic and health status,network reach and network diversity could result in the increased likelihood of using medical care service with the value of OR being 1.14(95%CI:1.01~1.39)and 1.34(95%CI:1.04~1.71),respectively.Meanwhile,network range did not pass the significant test.Conclusion Social network has a significant association with the use of medical care service among patients with cardiovascular disease.So,the public health strategies should be designed to facilitate the social participation,especially to develop the extension and reachability of social contact.
