外伤性肝破裂患者预后情况及影响因素研究分析

摘　　要：目的探究与分析外伤性肝破裂患者预后情况及影响因素。方法选取2006年4月—2017年4月本院收治的58例外伤性肝破裂患者作为研究对象。将其并发症发生率及死亡率进行统计,并比较不同年龄、性别、出血量、肝损伤分级、受伤至入院时间、合并其他部位损伤、合并基础疾病及心率者的并发症发生率及死亡率,并以多因素Logistic回归分析处理上述因素与外伤性肝破裂患者预后的关系。结果 58例外伤性肝破裂患者共发生13例并发症,发生率为22.41%,其中包括4例死亡病例,发生率为6.90%;不同性别患者的发生率比较,差异无统计学意义(P>0.05);不同年龄、出血量、肝损伤分级、受伤至入院时间、合并其他部位损伤、合并基础疾病及心率者的发生率比较,差异有统计学意义(P<0.05),多因素Logistic回归分析显示,年龄、出血量、肝损伤分级、受伤至入院时间、合并其他部位损伤、合并基础疾病及心率是外伤性肝破裂患者预后的影响因素。结论外伤性肝破裂患者预后的影响因素较多,可根据影响因素进行针对性干预。Objective To investigate and analyze the prognosis situation and its influence factors of patients with traumatic hepatorrhexis.Methods From April 2006 to April 2017,58 patients with traumatic liver rupture were selected as the research subjects.The incidence and mortality of complications were statistically analyzed.The incidence and mortality rate of different age,sex,bleeding volume,grade of liver injury,injury to admission,injury to other parts,combined with basic diseases and heart rate were compared.Multivariate Logistic regression analysis was used to analyze the relationship between these factors and prognosis of patients with traumatic liver rupture.Results 13 cases of complications occurred in 58 patients with traumatic liver rupture,the incidence rate was 22.41%,including 4 cases of death,the incidence was 6.90%.There was no significant difference in the incidence of different sex patients(P>0.05),and the difference of age,bleeding volume,grade of liver injury,injury to admission,injury to other parts,combined with basic diseases and heart rate were statistically significant(P<0.05).Multiple factor Logistic regression analysis showed that age,volume of bleeding,grade of liver injury,injury to admission time,combined injury of other parts,combined with basic disease and heart rate were the factors affecting the prognosis of patients with traumatic liver rupture.Conclusion There are many factors that influence the prognosis of patients with traumatic liver rupture.
