电针对高血脂合并脑缺血大鼠脑梗死体积及SVZ形态学影响

摘　　要：目的观察电针"百会""丰隆"对高血脂合并脑缺血大鼠脑梗死体积,以及脑组织形态学的影响,探讨"化痰开窍"配穴理论对高血脂合并脑缺血的可能影响机制。方法采用雄性SD大鼠78只,随机分为正常对照组(6只)、高血脂合并假手术组、高脂血症合并脑缺血模型组、高脂血症合并脑缺血电针1组和2组(每组18只)。除正常组外,其余各组大鼠喂养高脂饲料42天,电针1组电针"丰隆",每日1次,治疗7天。第50天,模型组、电针1、2组大鼠采用50% FeCl_3化学诱导大脑中动脉血栓闭塞制造高脂血症合并脑缺血模型。术后,电针1、2组电针"百会""丰隆"穴,每日1次,持续14天。用生物化学法检测血脂,用神经功能评分法评价行为学变化,用2,3,5-氯化三苯基四氮唑法检测脑梗死体积,进行苏木素-伊红染色,用光镜观察脑组织病理形态学变化。结果与正常组比较,模型组大鼠总胆固醇(total cholesterol,TC)、甘油三脂(triglycerides,TG)、低密度脂蛋白(low density lipoprotein,LDL)升高(P<0.05),高密度脂蛋白(high density lipoprotein,HDL)降低(P<0.05)。脑缺血术后出现神经功能缺损,患侧脑组织出现脑梗死,HE染色可见神经细胞坏死,炎性细胞浸润,侧脑室背外侧角(subventricular zone,SVZ)区神经细胞增殖。电针治疗可调节血脂,减小梗死体积(P<0.01),促进缺血后缺血皮层神经细胞恢复,促进SVZ区室管膜细胞增殖。高血脂阶段早期电针治疗效果优于脑缺血后电针治疗效果。结论高血脂阶段电针可通过调节血脂代谢,改善大鼠神经功能缺损状态,缩小梗死灶体积,促进神经细胞修复和增殖,实现对高血脂合并脑缺血脑组织保护的目的。Objective To observe the effects of electro-acupuncture at“Baihui”(GV20)and“Fenglong”(ST40)on cerebral infarction volume and the brain pathomorphology in hyperlipidemia rats with cerebral ischemia,and to explore the mechanism of matching acupoint theory of“Huatan Kaiqiao”.Methods 78 male SD rats were randomly divided into control(n=6),hyperlipidemia combined with sham operation group(sham),hyperlipidemia combined with middle cerebral Artery thrombus(hMCAT)group,EA1 group and EA2 group(n=18).Except the normal group,the other rats were fed with high-fat diet for 42 days.EA 1 group were electro-acupuncture at ST40 from 43 to 49 day.The rats were induced hMCAT model by 50%FeCl 3 on 50 day in model and EA groups.Then,GV20 and ST40 were needled in EA1 and 2 groups,1 times a day for 14 days.Serum lipids were measured by biochemistry method.The neurological function score was used to evaluate the changes of behavior.The volume of cerebral infarction was detected by 2,3,5-chlorotoluene tetrazolium chloride staining method.Pathomorphology was observed by hematoxylin-eosin staining method.Results Compared with the normal group,the levels of total cholesterol(TC),Triglycerides(TG)and low density lipoprotein(LDL)in the model group were increased significantly(P<0.05,P<0.01),while the high density lipoprotein was decreased(P<0.05,P<0.01).Rats had neurological function deficiency and cerebral infarction after hMCAT.HE staining showed nerve cells necrosis,inflammatory cell infiltration,and nerve cells proliferation in subventricular zone(SVZ)region.Electro-acupuncture can regulate blood lipid,reduce infarct volume(P<0.01),promote the recovery of ischemic cortical neurons after ischemia,and promote the proliferation of ependymal cells in SVZ area.The effect of electro-acupuncture at the early stage of hyperlipidemia was better than that of electro-acupuncture after cerebral thrombosis.Conclusion EA in the hyperlipemia stage can regulate the metabolism of blood lipid,improve the state of neurological deficit in rats,re
