HPA及NRF2与喉鳞状细胞癌发生和发展的相关性研究

摘　　要：目的探讨乙酰肝素酶(HPA)及核因子E2相关因子2(NRF2)在喉鳞状细胞癌中的表达及意义。方法收集2015年2月至2017年2月该院保存的喉鳞状细胞癌标本81份为病例组,同时选取77份声带息肉病理标本为对照组。采用免疫组织化学法检测HPA表达;Western blot检测NRF2表达。结果病例组和对照组的HPA阳性表达率和NRF表达量分别为81.48%、19.48%和0.844±0.113、0.202±0.094,差异有统计学意义(P<0.05);有淋巴结转移、TNM分期为Ⅲ~Ⅳ期患者HPA阳性表达率分别为93.02%和94.87%,明显高于无淋巴结转移和Ⅰ~Ⅱ期患者(P<0.05);中低分化患者HPA阳性表达率分别为93.75%和100.00%,明显高于高分化患者(P<0.05);有淋巴结转移、TNM分期为Ⅲ~Ⅳ期患者NRF2水平分别为0.901±0.122和0.885±0.105,明显高于无淋巴结转移和Ⅰ~Ⅱ期患者(P<0.05);中低分化患者NRF2水平分别为0.854±0.101和0.878±0.099,明显高于高分化患者(P<0.05)。结论喉鳞状细胞癌患者HPA及NRF2表达明显升高,与淋巴结转移、TNM分期及病理分级有关。Objective To investigate the expression of heparanase(HPA)and NF-E2 associated factor(NRF2)in laryngeal squamous cell carcinoma.Methods We collected 81 cases of laryngeal squamous cell carcinoma in our hospital from February 2015 to February 2017 as group A,and then selected 77 cases of polyp of vocal cord pathological specimens as group B.The expression of HPA was detected by immunohistochemistry,and the expression of NRF2 was detected by western blot.Results The positive expression rate of HPA and the expression of NRF in the group A and B were 81.48%,19.48%and 0.844±0.113,0.202±0.094,the difference was statistically significant(P<0.05).The positive expression rates of HPA in patients with lymph node metastasis and TNM stageⅢtoⅣwere 93.02%and 94.87%,which were significantly higher than those without lymph node metastasis and stageⅠtoⅡ(P<0.05).The positive expression rates of HPA in patients with low and middle differentiation were 93.75%and 100.00%,which were significantly higher than those with high differentiation(P<0.05).The expression of NRF2 in patients with lymph node metastasis and TNM stageⅢtoⅣwere 0.901±0.122 and 0.885±0.105,which significantly higher than those without lymph node metastasis and stageⅠtoⅡ(P<0.05).The expression of NRF2 in patients with moderate and low differentiation were 0.854±0.101 and 0.878±0.099,which were significantly higher than patients with stageⅠtoⅡ(P<0.05).Conclusion The expression of HPA and NRF2 in laryngeal squamous cell carcinoma are significantly increased,which are related to lymph node metastasis,TNM stage and pathological grade.
