胸腔内注射尿激酶治疗结核性胸膜炎患者临床效果分析

摘　　要：目的:探讨胸腔内注射尿激酶治疗结核性胸膜炎患者的临床效果。方法:选取河北省石家庄市第五医院结核科2014年12月~2017年12月明确诊断的结核性胸膜炎患者100例,分为A组(n=50)和B组(n=50);A、B组常规药物治疗,并行胸腔导管留置引流,A组于引流后,将10万U尿激酶注入患者胸腔内;B组则于引流后将0.1 g的异烟肼注入患者胸腔内。观察两组的治疗效果、导管留置时间、胸膜厚度、胸水吸收、胸膜粘连以及副反应情况。结果:A组显效35例,B组显效12例,两组总有效率比较,差异具有统计学意义(P<0.05);与B组比较,A组导管留置时间短(P<0.05),胸膜厚度较薄(P<0.05),胸水完全吸收数显著高(P<0.05),胸膜粘连数少(P<0.05),A组出现发热1例,2天后恢复。结论:胸腔内注射尿激酶治疗结核性胸膜炎患者临床治疗效果好,能有效改善患者临床病症,副反应少,安全性高。Objective:To investigate the clinical efficacy of intrapleural injection of urokinase on patients with tuberculous pleurisy.Methods:A total of 100 cases with tuberculous pleurisy admitted to Department of Tuberculosis of the Fifth Hospital of Shijiazhuang from December 2014 to December 2017 were selected and divided into A group(n=50)and B group(n=50).Based on the routine therapy and concurrent pleural catheter drainage,100 thousand U of urokinase were given to group A and 0.1 g intrapleural injection of isoniazid to B group.The therapeutic effect,the catheter indwelling time,the thickness of pleura,pleural fluid absorption,pleural adhesion and adverse reactions of the two groups were observed and compared.Results:The clinical effect of 35 cases was markedly effective in A group and 12 cases in B group,and the total efficiency of two groups was statistically significant difference(P<0.05).The catheter indwelling time in A group was shorter than that in B group(P<0.05).The pleural thickness in A group was thinner than that in B group(P<0.05).The water absorbed number in A group was significantly higher than that in B group(P<0.05).The number of pleural adhesions in A group was less than that in B group(P<0.05).1 case with fever in A group and after 2 days recovered.Conclusion:Intrapleural injection of urokinase in the treatment of patients with tuberculosis pleurisy can get good clinical curative effect and effectively relieve the clinical symptoms and has high safety with fewer side effects.
