替米沙坦联合川芎嗪治疗早期2型糖尿病合并高血压疗效观察

摘　　要：目的观察替米沙坦联合川芎嗪治疗早期2型糖尿病合并高血压患者的临床疗效。方法将进入实验的158例肾功能正常的早期2型糖尿病合并高血压患者随机分为对照组75例与观察组83例,2组患者均给予降压(替米沙坦)+降糖(二甲双胍+西格列汀)+改善循环(羟苯磺酸钙)治疗,观察组在此基础上给予川芎嗪治疗,连续治疗30d。观察2组治疗前后空腹血糖(FPG)、餐后2 h血糖(2h PG)、体质量指数(BMI)、收缩压(SBP)、舒张压(DBP)、血尿素氮(BUN)、血肌酐(Cr)、肾小球滤过率(GFR)、24 h尿微量白蛋白排泄率(AER)、尿白蛋白/肌酐比值(UACR)、血液流变学指标变化情况,并评估临床疗效,记录治疗过程中的不良反应。结果治疗后2组FPG、2h PG、SBP、DBP、BUN、Cr、GFR、AER、UACR、血液流变学指标均明显降低(P均<0.05),且观察组BUN、Cr、GFR、AER、UACR、血液流变学指标均明显低于对照组(P均<0.05),但2组间FPG、2h PG、SBP、DBP比较差异均无统计学意义(P均>0.05);治疗后2组BMI均无明显变化(P均>0.05)。观察组降压疗效明显优于对照组(P<0.05);治疗过程中2组均未发生严重不良反应,2组胃肠道反应、皮肤反应、低血糖、低血压发生率比较差异均无统计学意义(P均>0.05)。结论在早期肾功能正常的2型糖尿病合并高血压患者治疗中加用川芎嗪能明显提高降压效果,改善肾功能、血液流变学指标,且不良反应少。Objective It is to observe the clinical effect of telmisartan combinated with ligustrazine in patients with early type 2 diabetes mellitus(T2DM)with hypertension.Methods 158 patients with early T2DM and hypertension who had normal renal function were randomly divided into control group(n=68)and observation group(n=74).Both groups were given hypotensive(telmisartan)+hypoglycemic(melbinum+sitagliptin)+improving circulatory(ginaton)treatment,and the observation group were given ligustrazine on the basis of this therapy above,they were treated for 30 days.After treatment,the clinical effects were evaluated,the changes of fasting plasma glucose(FBG),2-hour postprandial blood glucose(2hPBG),body mass index(BMI),systolic blood pressure(SBP),diastolic blood pressure(DBP),blood urea nitrogen(BUN),creatinine(Cr),glomerular filtration rate(GFR),albumin excrete rate(AER),urinary albumin/creatinine ratio(UACR),blood rheology indexes were observed before and after treatment,the adverse events(AEs)during the treatment were recorded.Results After treatment,the clinical effect in the observation group were better than that in the control group(P<0.05);the FBG,2hPBG,SBP,DBP,BUN,Cr,GFR,AER,UACR,blood rheology indexes were all significantly lower than those before treatment in both groups(P<0.05),BUN,Cr,GFR,AER,UACR and blood rheology were lower in the observation group than those in the control group(P<0.05),but there was no significant difference in FPG,2hPG,SBP,DBP between the two groups(P>0.05);no obvious change of BMI was found in both groups(P>0.05).No serve AEs was found during the treatment,the incidences mainly included gastrointestinal events,skin-related effect had no statistical difference in the two groups(P>0.05).Conclusion Adding ligustrazine to the treat of T2DM with hypertension patients with normal renal function can improve hypotensive effect,renal function and blood rheology indexes with less AEs.
