中药苦参汤加减熏洗结合保留括约肌术治疗肛周脓肿40例疗效观察

摘　　要：目的探讨中药苦参汤加减熏洗结合保留括约肌术治疗肛周脓肿的临床疗效。方法将2016年3月—2017年3月因肛周脓肿于本院行手术治疗的80例患者纳入研究并随机分组,对照组40例术后行常规换药处理,观察组40例则联合中药苦参汤熏洗,14天一疗程。比较治疗前及治疗7、14d时的创面疼痛评分、创面肉芽评分、创面缩小率情况,比较临床疗效差异。结果治疗后观察组7、14d时的创面疼痛评分、创面肉芽评分更低,创面缩小率更高,差异显著,P<0.05;观察组总有效率95.00%,高于对照组的77.50%,差异显著,P<0.05。结论中药苦参汤加减熏洗结合保留括约肌术治疗肛周脓肿效果显著,可有效促进术后创口修复,值得推广。To explore clinical outcome of modified fumigation and washing of herbal sophora decoction combining with sphincter preservation surgery in treating perianal abscess.Methods:80 patients undergoing surgery for perianal abscess from March 2016 to March 2017 were included in the study and randomly divided into two groups.After surgery,40 patients in the control group were performed with conventional dressings while 40 patients in the observation group were fumigated and washed with herbal sophora decoction for 14 days.Wound pain scores,granulation tissue scores,wound size reduction rate and clinical outcome rate were assessed at day 0,7 and 14 post-treatment.Results:Wound pain scores,granulation tissue scores and wound size reduction rate were significantly mitigated by herbal sophora decoction in the observation group(P<0.05).The clinical outcome rate was 95%(38/40)in the observation group and 77.50%(31/40)in the control group with significant difference(P<0.05).Conclusion:Fumigation-washing of Herbal sophora decoction after sphincter preservation surgery has remarkable effects on perianal abscess,which can effectively improve postoperative wound repair and be worthy of application.
