全膝关节置换术前焦虑对术后疼痛及麻醉恢复的影响

摘　　要：目的探讨全膝关节置换术前焦虑对术后疼痛及麻醉恢复的影响。方法选取2016年12月—2017年12月在我院接受治疗的100例全膝关节置换术患者作为本次研究对象,并在手术之前,通过贝克焦虑量表,将入选患者分为严重焦虑组(56例)与普通焦虑组(44例),并对两组患者麻醉时间、手术时间以及拔管时间进行对比,同时还对两组患者的麻醉后的不良反应、曲马多用量以及术后视觉评分进行对比。结果两组在麻醉时间以及手术时间方面对比,差异无统计学意义(P>0.05);普通焦虑组在拔管时间、麻醉后的不良反应、曲马多用量以及术后视觉评分均优于严重焦虑组,两组对比,差异具有统计学意义(P<0.05)。结论通过全膝关节置换术治疗的患者,其术后疼痛及麻醉恢复与术前焦虑存在密切的关联,术前焦虑程度过高不利于患者术后疼痛及麻醉的恢复,还会增加药物的使用量。Objective To investigate the effect of preoperative anxiety on postoperative pain and anesthesia recovery in total knee replacement.Methods 100 cases of total knee arthroplasty in our hospital from December 2016 to December 2017 were selected as the study subjects.Before the operation,the Beck anxiety scale was used to divide the patients into the severe anxiety group(56 cases)and the general anxiety group(44 cases).The time of anesthesia,the time of operation and the time of extubation were compared in the two groups.The adverse reactions after anesthesia,the dose of tramadol and the postoperative visual score were also compared in the two groups.Results There was no significant difference between the two groups in the time of anesthesia and the time of operation(P>0.05).The time of extubation,postoperative adverse reactions,tramadol dosage and postoperative visual score in the general anxiety group were better than those in the severe anxiety group.The difference between the two groups was statistically significant(P<0.05).Conclusion After total knee arthroplasty,postoperative pain and recovery are closely related to preoperative anxiety.High preoperative anxiety is not conducive to postoperative pain and anesthesia recovery,but also increases drug usage.
