摘 要:目的:应用MRI基于体素的形态学分析法(VBM)评估轻度尼古丁依赖者脑灰质结构的改变。方法:使用Siemens Skyro 3.0T磁共振扫描仪对77例轻度尼古丁依赖者(FTND评分<6分)和37例不吸烟的健康志愿者进行扫描,包括常规MRI序列及高分辨三维T1加权磁化梯度回波序列(3DMPRAGE,结构像)。结构像数据采用基于Matlab平台的SPM8软件进行数据处理和分析,对轻度尼古丁依赖组与对照组的灰质体积进行两独立样本t检验(FWE校正,P<0.05,簇体素>318voxels),将两组间灰质体积(GMV)差异区与吸烟情况进行Spearman相关分析。结果:与对照组比较,轻度尼古丁依赖组双侧丘脑和双侧缘上回的GMV减小,未发现GMV增大的脑区。右侧和左侧丘脑、右侧缘上回的GMV与吸烟指数呈负相关(r=-0.489、-0.500和-0.312,P<0.05),与吸烟年限也呈负相关(r=-0.497、-0.555和-0.351,P<0.05),右侧丘脑GMV还与日吸烟量呈负相关(r=-0.260,P<0.05)。结论:吸烟者脑灰质结构的改变主要集中在奖励相关通路及边缘系统,且脑灰质体积与吸烟的累积量相关。Objective:To evaluate brain structure changes in mild nicotine dependence by using MRI and voxel-based morphometry(VBM)study.Methods:77 mild nicotine dependence individuals(FTND<6 score)and 37 non-smoking healthy volunteers were scanned with Siemens Skyro 3.0T magnetic resonance scanner.All subjects underwent routine sequence(to exclude intracranial lesions)and 3D-MPRAGE sequence MRI examination.SPM8 software based on Matlab was used to analyze the data.The independent sample t test(FWE correction,P<0.05,cluster size>318 voxels)was used to compare the grey matter volume(GMV)between the smoking group and the control group,and then the Spearman correlation analysis was used to analyze the correlation of the GMV with statistical difference compared with control group and the smoking related indexes.Results:Compared with the control group,the mild nicotine dependent group exhibited decreased GMV in bilateral thalamus and bilateral supramarginal gyrus,and the area with elevated GMV was not found.The GMV of right thalamus,left thalamus and right supramarginal gyrus was significant negatively correlated with smoking index(r=-0.489,-0.500 and-0.312,P<0.05),and also negatively correlated with smoking years(r=-0.497,-0.555 and-0.351,P<0.05),and the GMV of right thalamus was also negatively correlated with the number to daily smoking(r=-0.260,P<0.05).Conclusion:The changes of brain structure of smokers mainly focused on reward related pathways and marginal systems,and the gray matter volume of certain brain regions is related to smoking behavior data.
