摘  要:目的对比研究结核性胸膜炎患者血清及胸腔积液中细胞因子INF-γ、IL-2、IL-4、IL-5和IL-10的含量,探讨结核性胸膜炎患者的免疫状况,提高对结核性胸膜炎的诊断水平。方法选取20~45岁年龄段结核性胸膜炎患者50例作为结核性胸膜炎组,非结核性胸膜炎患者15例为非结核性胸膜炎组,健康人群15例为健康对照组,应用流式细胞仪检测结核性胸膜炎、非结核性胸膜炎患血清、胸腔积液及健康人群血清中INF-γ、IL-2、IL-4、IL-5、IL-10的含量。结果结核性胸膜炎与非结核性胸膜炎胸腔积液INF-γ、IL-2、IL-4、IL-5、IL-10的含量:结核性胸膜炎组为(448.96±191.97)pg/ml、(70.07±24.95)pg/ml、(87.00±12.25)pg/ml、(670±130.14)pg/ml、(398±120.11)pg/ml;非结核性胸膜炎组为(47.77±27.67)pg/ml、(33.42±18.84)pg/ml、(48±15.55)pg/ml、(385±119.35)pg/ml、(146±189.75)pg/ml,2组差异有统计学意义(P<0.05)。结核性胸膜炎、非结核性胸膜炎及健康人群血清中INF-γ、IL-2、IL-4、IL-5、IL-10的含量:结核性胸膜炎组为(42.63±18.35)pg/ml、(31.31±19.23)pg/ml、(49.00±20.35)pg/ml、(367±118.41)pg/ml、(137±102.47)pg/ml;非结核性胸膜炎组为(38.43±18.80)pg/ml、(30.27±19.24)pg/ml、(49.2±18.35)pg/ml、(380±119.35)pg/ml、(141±101.75)pg/ml;健康人群组为(39.19±17.96)pg/ml、(39.39±21.96)pg/ml、(49.5±20.75)pg/ml、(379±117.95)pg/ml、(139±101.35)pg/ml,3组两两比较差异均无统计学意义(P>0.05)。结论通过检测胸腔积液中INF-γ、IL-2、IL-4、IL-5、IL-10等细胞因子的含量,为结核性胸膜炎的诊断及鉴别诊断提供可靠依据。Objiecive To compare the levels of cytokines-IFN-γ,IL-2,IL-4,IL-5 and IL-10 in serum and pleural effusion of patients with tuberculous pleurisy,and to investigate the cellular immune status of patients in order to improve the levels of the diagnosis of tuberculous pleurisy.Methods Fifty patients with tuberculous pleurisy(age range:20~45yr)were enrolled as tuberculous pleurisy group,and the other 15 patients with non-tuberculous pleurisy were enrolled as non-tuberculous pleurisy group,moreover 15 healthy subjects were enrolled as control group.The levels of IFN-γ,IL-2,IL-4,IL-5 and IL-10 in serum and tuberculous pleurisy were detected by flow cytometry.Results There were significant differences in the levels of IFN-γ,IL-2,IL-4,IL-5 and IL-10 in pleural effusion between tuberculous pleurisy group and non-tuberculous pleurisy group(P<0.05).However there were np significant differences in the levels of The levels of IFN-γ,IL-2,IL-4,IL-5 and IL-10 in serum among the three groups each other(P>0.05).Conclusion It can provide reliable references for clinical diagnosis and differential diagnosis of tuberculous pleurisy to detect the levels of IFN-γ,IL-2,IL-4,IL-5 and IL-10 in pleural effusion of patients.
