经肛肠梗阻导管及自扩张金属支架治疗左半结肠癌性梗阻的研究进展

摘　　要：左半结肠癌性梗阻的手术风险较大,近年来,随着技术手段的发展,普遍建议行Ⅰ期切除吻合术,经肛肠梗阻导管和自扩张金属肠道支架在治疗左半结肠癌性梗阻中的应用日益增加。经肛肠梗阻导管可缓解局部的梗阻症状,争取行Ⅰ期切除吻合术手术的时间,还可进行结肠灌洗,为手术行充分的肠道准备,并在术后配合药物灌肠等辅助疗法,对术后粘连性肠梗阻等并发症起到治疗作用。而自扩张金属肠道支架适合年龄较大、不宜手术患者,在采用其他方法度过肠梗阻急性期后缓解肠梗阻症状,并结合化疗等保守治疗。The risk of the surgery for left semicolon cancer combined with obstruction is high.Simultaneous stageⅠexcision and anastomosis is generally regarded to be better with the increasing application of intestinal tract catheter and self-expanding metallic stent.Intestinal tract catheter can be used in the preparation of the surgery by relieving the symptom and clean the colon through irrigation.It can also deal with postoperative complications such as intestinal adhesion.Self-expanding metallic stent can be placed after acute stage to relieve the obstruction symptom and to be applied as an interim to following expectant treatment such as chemotherapy.
