老年人臂踝脉搏波速度的变化及其相关因素分析

摘　　要：目的探讨老年人臂踝脉搏波速度(ba PWV)的变化及其相关影响因素。方法选取2016年3月至10月我院体检的60岁以上老年人965例,其中男性512例,女性453例,年龄(651±6)岁。所有受检者测量体质量指数(BMI)、血压、静息心率,检测空腹血糖(FPG)、甘油三酯(TG)、总胆固醇(TC)、高密度脂蛋白(HDL)、低密度脂蛋白(LDL),测定ba PWV,根据公式计算最大吸氧量(VO2_(max))。结果将受检者按照ba PWV水平四分位分组,分别为Q1、Q2、Q3、Q4组。Q4组年龄高于其余3组,收缩压(SBP)水平随着ba PWV水平的增高,逐渐增加。Q4组VO2_(max)水平低于其余3组。Q4组TG高于其他3组,Q4组TC水平高于Q1、Q3组。ba PWV水平与SBP、DBP、BMI、FPG、TG、TC、LDL、年龄呈正相关(r=0.678、0.430、0.172、0.222、0.214、0.144、0.145、0.329,P<0.05),与VO2_(max)呈负相关(r=-0.122,P<0.05)。多元逐步回归分析结果显示SBP、年龄、BMI、TG是ba PWV水平的独立危险因素(β=0.675,0.233,-0.107,0.094,P<0.05)结论 SBP、DBP、BMI、FPG、TG、TC、LDL、年龄、VO2_(max)与老年人ba PWV水平明显相关,其中SBP、年龄、BMI、TG是老年人ba PWV水平的独立危险因素。Objective To investigate the varience and related factors of brachial-ankle pulse wave velocity(baPWV)in the elderly.Methods Nine hundred and sixty five patients 512 male,453 female and average age 65±6 older than 60 years were selected who underwent health check-up from March to December in 2016 in our hospital.All subjects were inquired about their body mass index(BMI),blood pressure,resting heart rate,fasting plasma glucose(FPG),triglyceride(TG),total cholesterol(TC),high density lipoprotein(HDL),low density lipoprotein(LDL),baPWV as well as the maximum oxygen uptake(VO 2max)were also calculated according to the formula.Participants were grouped according to the baPWV quartile,respectively Q1,Q2,Q3,Q4 four groups.Results The age and TG of Q4 group was higher than those of the other three groups and the level of VO 2max in Q4 group was lower than those of the other three groups.The SBP increased gradually with the increasing of baPWV.The TC level in Q4 group were higher than that of Q1 and Q3 groups.The baPWV were positively correlated with SBP,DBP,BMI,FPG,TG,TC,LDL and age(R=0.678,0.430,0.172,0.222,0.214,0.144,0.145,0.329,P<0.05)and negative correlated with VO 2max(r=-0.122,P<0.05).The SBP,age,BMI,and TG were independent risk factors for baPWV(β=0.675,0.233,-0.107,0.094,P<0.05)showed by further progressive regression analysis.Conclusion SBP,DBP,BMI,FPG,TG,TC,LDL,age,VO 2max were significantly correlated with the level of baPWV in the elderly and SBP,age,BMI and TG were independent risk factors for baPWV in the elderly.
