摘  要:目的观察清毒提取液对脓毒症大鼠血清肿瘤坏死因子α(TNF-α)、高迁移率族蛋白1(HMGB1)的干预作用和对大鼠生存率的影响,探讨清毒提取液治疗脓毒症的机制。方法将90只健康雄性Wistar大鼠随机分为假手术组(n=10)、模型组(n=10)、乌司他丁组(n=10)及清毒提取液低、中、高剂量组(每组n=20)。采用盲肠结扎穿孔法制作脓毒症大鼠模型(假手术组只打开腹腔,不盲肠造瘘),于盲肠结扎穿孔术前1 h和术后24、48、72、84 h分别给各组大鼠相应干预,假手术组、模型组予0.9%氯化钠注射液灌胃,乌司他丁组予注射用乌司他丁尾静脉注射,清毒提取液低、中、高剂量组大鼠分别给予清毒提取液4.25、8.5、17 m L/kg清毒提取液灌胃。造模后96 h存活大鼠于股动脉处取血,采用酶联免疫吸附法(ELISA)测定各组大鼠血清TNF-α、HMGB1水平,并统计死亡率。结果造模后,模型组大鼠出现精神萎靡等状况,乌司他丁组及清毒提取液低、中、高剂量组大鼠精神状态优于模型组。模型组、乌司他丁组及清毒提取液低、中、高剂量组死亡率均高于假手术组(P<0.05);乌司他丁组及清毒提取液低、中、高剂量组死亡率均低于模型组(P<0.05);清毒提取液中、高剂量组死亡率低于乌司他丁组及清毒提取液低剂量组(P<0.05)。模型组、乌司他丁组及清毒提取液低、中、高剂量组HMGB1均高于假手术组(P<0.05);模型组、乌司他丁组及清毒提取液低、中剂量组TNF-α均高于假手术组(P<0.05);乌司他丁组及清毒提取液低、中、高剂量组HMGB1、TNF-α均低于模型组(P<0.05);乌司他丁组及清毒提取液低、中、高剂量组HMGB1两两比较差异无统计学意义(P>0.05);清毒提取液低、中剂量组TNF-α与乌司他丁组比较差异无统计学意义(P>0.05),清毒提取液高剂量组TNF-α低于乌司他丁组及清毒提取液低、中剂量组(P<0.05)。结论清毒提取液能降低脓毒症大鼠�Objective To observe the effects of clear toxin liquid on serum tumor necrosis factor-alpha(TNF-a)and high mobility group protein-1(HMGB1)in rats with sepsis and the effect on the survival rate of rats,and to explore the mechanism of the treatment of sepsis.Methods 90 healthy male Wistar rats were randomly divided into sham operation group(n=10),model group(n=10),ulastatin group(n=10)and clear toxin liquid low,medium and high dose group(n=20).The sepsis rat model was made by cecal ligation and puncture(the sham group only opened the abdominal cavity and no caecal fistula).One hour before cecal ligation and puncture,and 24,48,72,and 84 hours after operation,the rats in each group were given appropriate interventions.The sham operation group and the model group were given 0.9%sodium chloride injection by gavage,and the ulstastin group was injected by the intravenous injection of ulstachin in rat tail vein.The clear toxin liquid low,medium and high dose group were given 4.25,8.5 and 17 mL/kg separately by gavage.After 96 hours of model establishment,blood samples were collected from femoral artery,TNF-αand HMGB1 levels were measured by enzyme-linked immunosorbent assay(ELISA),and mortality was calculated.Results After the model was established,the rats in the model group were mentally depressed,and the mental state of the rats in the low,middle and high dose of the clear toxin liquid groups and ulastatin group was better than that of the model group.The mortality rates of the model group,the ulastatin group,and the low,middle,and high doses of the clear toxin liquid group were higher than those of the sham operation group(P<0.05).The mortality rates of ulastatin group and low,middle and high dose of the clear toxin liquid groups were lower than those of the model group(P<0.05).The mortality rate of the middle and high dose groups of the clear toxin liquid group were lower than that of the ulastatin group and the low-dose group of the clear toxin liquid(P<0.05).The HMGB1 levels in the model group,the ulastatin group
