温肾健脾方对重症肌无力患者免疫功能的影响

摘　　要：目的观察温肾健脾方对重症肌无力(MG)脾肾阳虚型患者外周血B淋巴细胞亚群、白细胞介素-6(IL-6)、B淋巴细胞活化因子(BAFF)表达的影响。方法入选脾肾阳虚型MG患者18例为观察组,同期健康体检者20例为正常对照组,采用ELISA法检测治疗前、治疗后IL-6、BAFF的水平,采用流式细胞术检测治疗前、后外周血B淋巴细胞亚群比率。结果 MG患者IL-6、BAFF表达明显高于正常对照组(P<0.01),温肾健脾方治疗1月后,MG患者IL-6、BAFF水平明显下降,差异有统计学意义(P<0.01);MG患者治疗前、后CD19+CD27+细胞百分比分别为(1.04±0.24)%、(0.96±0.28)%,差异无统计学意义(P>0.05),CD19+CD27-细胞百分比分别为(3.02±1.12)%、(2.12±0.42)%,差异有统计学意义(P<0.05)。结论温肾健脾方对MG患者有一定的免疫调节作用,其机制可能与降低外周血中未分化B淋巴细胞数量及BAFF、IL-6水平有关。Objective To observe the effect of Wenshen Jianpi decoction on the immunologic function in spleen-kidney yang deficiency patients with myasthenia gravis.Methods 18 MG patients with spleen-kidney yang deficiency were selected as observation group and 20 healthy subjects were selected as normal control group in the same period.The levels of BAFF and IL-6 were measured by ELISA and the percentage of B lymphocyte subsets in peripheral blood was detected by flow cytometry before and after treatment.Results The expression of IL-6,BAFF in MG patients were significantly higher than the normal control group(P<0.01).After Wenshen Jianpi decoction treatment for 1 month,the levels of IL-6 and BAFF in MG patients decreased significantly(P<0.01).The percentage of CD19+CD27+cells before and after treatment were(1.04±0.24)%,0.96±0.28)%,the percentage of CD19+CD27-cells before and after treatment were(3.02±1.12)%,(2.12±0.42)%,the differences were ststistically significant(P<0.05).Conclusion Wenshen Jianpi decoction has exact effects on immune regulation in patients with myasthenia gravis,and the mechanism may be related to reducing the number of undifferentiated B lymphocytes and the levels of BAFF and IL-6 in peripheral blood.
