摘　　要：目的探讨不同CO_2气腹压力对腹腔镜胆囊切除术患者应激反应的影响。方法选取2014年9月至2017年9月我院收治的胆囊结石老年患者100例为研究对象,均采取腹腔镜胆囊切除术治疗。将患者随机分为观察组(50例)和对照组(50例),观察组CO_2气腹压力为10~13mm Hg,对照组为14~16mm Hg,比较两组患者的心率、血压及应激反应状况。结果 CO_2气腹建立后30分钟,两组患者的HR,MAP,Pa CO_2,Cor,IL-6,CRP以及Glu等指标均显著上升,且观察组的上升幅度明显小于对照组,分别比较差异显著(P<0.05或P<0.01)。结论低CO_2气腹压力可显著降低腹腔镜胆囊切除术患者的应激反应,提高手术安全性,值得临床应用推广。To investigate the effects of different CO 2 pneumoperitoneum pressure on stress response in patients undergoing laparoscopic cholecystectomy.Methods A total of 100 patients with cholecystolithiasis were enrolled in our hospital from September 2014 to September 2017,all patients underwent laparoscopic cholecystectomy.The patients were randomly divided into observation group(50 cases)and control group(50 cases).The observation group was 10~13mmHg pneumoperitoneum pressure,14~16mmHg in the control group,compared the heart rate,blood pressure and stress response of the two groups.Results After the establishment of CO 2 pneumoperitoneum 30min,the HR,MAP,PaCO 2,Cor,IL-6,CRP and Glu indexes of the two groups increased significantly,the increase of observation group was significantly lower than that in the control group,the difference was significant(P<0.05or P<0.01).Conclusion Low CO 2 pneumoperitoneum pressure can significantly reduce laparoscopic cholecystectomy in patients with stress response,improve surgical safety,worthy of clinical application to promote.
