摘　　要：目的探讨慢性乙型肝炎(CHB)患者的肾功能损害情况及其危险因素。方法选择未行抗病毒治疗的CHB患者410例,应用简化肾脏病膳食改良公式计算肾小球滤过率估计值(e GFR),评估肾功能损害情况。采用多因素logistic回归模型分析CHB患者肾功能损害的相关影响因素。结果 CHB患者肾功能损害发生率为19.76%(81/410)。多因素分析结果显示,男性和年龄>50岁是CHB患者发生肾功能损害的独立危险因素(均P<0.05)。结论部分CHB患者可于抗病毒治疗前发生肾功能损害,其中男性及年龄>50岁患者发生风险更高。Objective To investigate the renal damage and its risk factors in patients with chronic hepatitis type B(CHB).Methods A total of 410 patients with CHB without antiviral treatment were enrolled in this study.The estimated glomerular filtration rate(eGFR)calculated by Modification of Diet in Renal Disease(MDRD)was used to assess renal function damage.The related influencing factors of renal function damage were analyzed by multivariate logistic regression model.Results The incidence rate of renal function damage was 19.75%(81/410)in CHB patients.The results of multivariate analysis showed that male and age>50 years were the independent risk factors for renal function damage in CHB patients(all P<0.05).Conclusions Some CHB patients might suffer renal function damage before antiviral therapy.Male patients or patients older than 50 years have a higher risk for renal function damage.
