乳管镜下选择性乳管冲洗治疗急性期及亚急性期浆细胞性乳腺炎的疗效评价

摘　　要：目的分析急性期及亚急性期浆细胞性乳腺炎治疗中乳管镜下选择性乳管冲洗治疗的应用效果。方法以80例浆细胞性乳腺炎患者作为观察对象,随机将患者分为两组,对照组接受药物保守治疗,观察组接受乳管镜下选择性乳管冲洗治疗,每组40例。分析两组治疗效果及治疗不良反应发生率。结果 2周治疗完成后,两组治疗总有效率比较差异无统计学意义(P>0.05),而观察组治愈率高于对照组,差异有统计学意义(P<0.05)。2周治疗时间内,观察组治疗不良反应发生率低于对照组,差异有统计学意义(P<0.05)。治疗过程中,两组泌乳素均呈降低趋势,转化生长因子-β(TGF-β)均呈升高趋势,而治疗1周及2周时,观察组泌乳素均低于对照组,TGF-β高于对照组(P<0.05)。结论相有对于药物保守治疗,乳管镜下选择性乳管冲洗治疗可显著的提高急性期及亚急性期浆细胞性乳腺炎的临床治疗效果。Objective To analyze the application effect of selective breast lavage therapy in the treatment of plasma cell mastitis in acute and subacute stage.Methods Eighty patients with plasma cell mastitis were selected as the observation subjects.They were randomly divided into two groups.The control group received conservative treatment,while the observation group received selective breast lavage treatment,with 40 cases in each group.The therapeutic effect and incidence of adverse reactions of the two groups were analyzed.Results After 2 weeks of treatment,the total effective rate of the two groups had no statistical difference(P>0.05),but the cure rate of the observation group was higher than that of the control group,with statistical difference(P<0.05).During the 2 week treatment time,the incidence of adverse reactions in the observation group was lower than that in the control group,with statistical difference(P<0.05).During the treatment,prolactin of the two groups all decreased,and the transforming growth factor-beta(TGF-beta)were all increasing,while the prolactin in the observation group was lower than that in the control group at the 1 week and 2 weeks,and the TGF-beta was higher than that of the control group(P<0.05).Conclusion Compared with conservative treatment,selective lactoduct irrigation under endoscopy can significantly improve the clinical efficacy of acute and subacute stage of plasma cell mastitis.
