摘　　要：目的探究单绒毛膜囊双羊膜囊双胎脐带胎盘附着给予产前超声监测预测妊娠结局的临床应用价值。方法选取本院2013年1月~2016年1月诊断为单绒毛膜囊双羊膜囊双胎孕妇50例作为研究对象,按照双胎脐带胎盘附着是否一致分为一致组(24例)和不一致组(26例)。回顾50例患者产前超声检查结果和产后临床资料,对比两组患者的复杂性双胎发生率。结果不一致组复杂性双胎发生率为57.69%明显高于一致组16.67%,组间比较差异有统计学意义(P<0.05);不一致组静脉导管异常、脐动脉血流异常、羊水过少占比例均高于一致组,组间比较差异有统计学意义(P<0.05);帆状附着双胎输血综合征发生率(28.57%)高于边缘附着(0),比较差异有统计学意义(P<0.05)。结论产前超声检查可对脐带胎盘附着不一致的单绒毛膜囊双羊膜囊双胎孕妇提供妊娠结局预测。Objective To explore the clinical value of prenatal ultrasound monitoring and predicting pregnancy outcome by single chorionic sac twin amniotic sac twin umbilical cord placenta attachments.Methods From January 2013 to January 2016,50 pregnant women diagnosed as single chorionic sac twin amniotic sac twin were selected as the research objects.According to the consistency of umbilical cord placental attachment of twin fetuses,they were divided into two groups:concordance group(26 cases)and inconsistency group.The results of prenatal ultrasound and postnatal clinical data of 50 patients were reviewed.The incidence of complex twins was compared between the two groups.Results The incidence of complex twins in inconsistent group was 57.69%,which was significantly higher than that in consistent group(16.67%).the difference between the two groups was statistically significant(P<0.05).The proportion of venous catheter abnormality,umbilical artery blood flow abnormality and oligohydramnios in the inconsistent group was higher than that in the consistent group,and the difference between the two groups was statistically significant(P<0.05).The incidence of veligform twin transfusion syndrome(28.57%)was higher than that of marginal attachment(0),and the difference was statistically significant(P<0.05).Conclusion Prenatal ultrasonography can predict pregnancy outcomes in women with single chorionic sac twin amniotic sac twin with inconsistent umbilical cord placental attachment.
