G试验和GM试验联合检测对肺侵袭性真菌病的诊断价值

摘　　要：目的探讨G试验和GM试验联合检测对肺侵袭性真菌病的诊断价值。方法选取2016年8月~2017年8月在本院拟诊为肺侵袭性真菌病患者60例为观察对象,同时选取非感染者60例作为对照组。对其支气管肺泡灌洗液(BALF)和血清半乳甘露聚糖(GM)、血清(1,3-β)-葡聚糖(1,3-beta-D-glucan,G test)进行检测。观察上述指标对肺侵袭性真菌病的诊断价值。结果肺侵袭性真菌感染组患者的血清GM、BALF GM和血清G试验实验结果高于对照组(t=-7.390、-16.462、-84.653,P<0.001);血清GM、BALF GM和血清G试验联合检测的特异性和灵敏度明显高于单项检测;肺侵袭性真菌病患者的WBC、NEU%和CRP水平与血清GM、BALF GM和血清G试验单相和联合检测的准确性正相关。结论 G试验和GM试验联合检测对肺侵袭性真菌病的诊断价值较高,可作为临床重要的辅助诊断指标。Objective To investigate the diagnostic value of G test and GM test in pulmonary invasive fungal disease.Methods Suspected pulmonary invasive fungal disease patients treated in our hospital from Aguest 2016 to Aguest 2017 were selected as the observation object,at the same time non infected persons were selected as control group.Bronchoalveolar lavage fluid(BALF)and serum galactomannan(GM),serum(1,3-beta)-glucan(1,3-beta-D-glucan,G)were detected.To observe the diagnostic value of the above indexes in invasive pulmonary mycosis.Results The results of serum GM,BALF GM and serum G in patients with invasive fungal infection were higher than those in the control group(t=-7.390,-16.462,-84.653,P<0.001);the specificity and sensitivity of combined detection of serum GM,BALF GM and serum G were significantly higher than that of single detection;the levels of WBC,NEU%and CRP were positively correlated with the serum GM,BALF GM and the accuracy of single phase and combined detection of serum G,serum levels of the patients with invasive fungal disease.Conclusion The combined detection of G and GM is valuable for the diagnosis of invasive pulmonary fungal disease,and can be used as an important auxiliary diagnostic index.
