摘 要:目的探究万托林、异丙托溴铵及布地奈德联合治疗对慢性阻塞性肺疾病(COPD)的患者动脉血气及肺功能的影响。方法选择94例COPD患者,根据治疗方式不同分为两组,每组47例。对照组予以布地奈德治疗,2次/d;试验组在对照组治疗基础上予以雾化吸入万托林和异丙托溴铵治疗。治疗1周后,比较两组临床疗效、肺功能、动脉血气指标及不良反应发生情况。结果试验组治疗总有效率为95.74%(45/47)显著高于对照组的82.98%(39/47),差异有统计学意义(P<0.05);试验组动脉血二氧化碳分压(Pa CO2)为(45.29±6.58)mm Hg低于对照组的(53.87±6.14)mm Hg,试验组动脉血氧分压(Pa O2)为(68.26±11.47)mm Hg、第1秒用力呼气末容积(FEV1)为(1.62±0.24)L、FEV1与用力肺活量比值(FEV1/FVC)为(69.84±6.42)%,对照组Pa O2为(61.59±10.55)mm Hg、FEV1为(1.16±0.38)L、FEV1/FVC为(59.46±7.08)%,组间比较差异有统计学意义(P<0.05);两组不良反应总发生率相比,差异无统计学意义。结论对COPD患者实施万托林、异丙托溴铵联合布地奈德治疗,能有效改善患者肺功能,提高临床疗效,且不会增加毒副反应,安全性较高。Objective To investigate the effect of ventolin,ipratropium bromide and budesonide on arterial blood gas and pulmonary function in patients with chronic obstructive pulmonary disease(COPD).Methods 94 COPD patients were divided into two groups by treatment methods,47 cases in each group.Control group was given budesonide,twice a day;experimental group was treated with aerosol inhalation of ventolin combined with ipratropium bromide based on control group.After one-week treatment,the clinical efficacy,pulmonary function,arterial blood gas indexes,adverse reactions were compared between the two groups.Results The total effective rate of experimental group was 95.74%(45/47),which was higher than 82.98%(39/47)of control group(P<0.05);The partial pressure of carbon dioxide in artery(PaCO2)in experimental group was(45.29±6.58)mmHg,which was lower than(53.87±6.14)mmHg in control group,the arterial partial pressure of oxygen(PaO2),forced expiratory volume in 1 second(FEV1),FEV1/forced vital capacity(FEV1/FVC)in experimental group were(68.26±11.47)mmHg,(1.62±0.24)L,(69.84±6.42)%,the PaO2,FEV1,FEV1/FVC in control group were(61.59±10.55)mmHg,(1.16±0.38)L,(59.46±7.08)%,and there was statistical difference(P<0.05);There was no statistical difference in the incidence of adverse reactions between the two groups.Conclusion Ventolin,ipratropium bromide and budesonide in treatment of COPD can improve pulmonary function,increase clinical efficacy and not add toxic-side effects,which has higher safety.
