微创保胆取石术与腹腔镜胆囊切除术治疗胆结石的临床疗效及预后观察

摘　　要：目的观察胆结石应用微创保胆取石术与腹腔镜胆囊切除术治疗的临床疗效及预后。方法选取128例胆结石患者临床资料,按术式不同分两组,各组64例,对照组行腹腔镜胆囊切除术,观察组行内镜微创保胆取石术,比较组间手术指标及并发症、生存质量。结果观察组手术时间、首次排气时间及住院时间均比对照组短,术中出血量比对照组少;观察组术后并发症发生率3.13%比对照组23.44%低(P<0.05);观察组术后3个月的GLQI总分(105.26±8.96)分比对照组高(P<0.05)。结论微创保胆取石术、腹腔镜胆囊切除术清除胆结石的效果相当,但微创保胆取石术的安全性高、预后显著,可缩短手术时间,减少手术出血量,加快胆结石患者术后恢复,减少术后并发症,提高生存质量。Objective To observe the curative effect and prognosis of gallstones treated with minimally invasive cholecystectomy and laparoscopic cholecystectomy.Methods The clinical data of 128 patients with gallstones were selected according to different surgical methods.There were 64 cases in each group.The control group underwent laparoscopic cholecystectomy.The observation group underwent endoscopic minimally invasive cholecystectomy.The surgical indexes were compared between the two groups.Complications,quality of life.Results Compared with the control group,the operative time,first time of evacuation and length of stay in the observation group were shorter.The amount of blood loss was less in the observation group than in the control group.The incidence rate of postoperative complications in the observation group was 3.13%lower than that in the control group(23.44%)(P<0.05).The total score of GLQI(105.26±8.96)was higher in the observation group than in the control group at 3 months after operation(P<0.05).Conclusion Minimally invasive gallbladder lithotripsy and laparoscopic cholecystectomy are effective in removing gallstones,but minimally invasive gallbladder lithotripsy is safe and has a significant prognosis.It can shorten the operation time,reduce the amount of blood loss,and speed up the gallstone patients.After recovery,reduce postoperative complications and improve quality of life.
