摘 要:目的:通过自动听性脑干反应仪对早产儿听力进行筛查,分析患儿听力损伤发生情况,从而早期发现、诊断听力障碍,及早干预。方法:随机选取我院产科收治的250例早产儿,使用美国ALGO3i自动脑干诱发电位(AABR)对其进行听力检测,并对未通过听力检测的早产儿在30 d后再次行听力检测,对比分析筛查结果、复筛结果以及1年后随访情况。结果:胎龄28~32 w早产儿初筛听力检测通过率(66.67%)与胎龄34~36 w早产儿(87.50%)相比较低(P<0.05)。合并其他疾病早产儿初筛听力检测通过率(75.38%)与未合并其他疾病早产儿(87.50%)相比较低(P<0.05)。胎龄28~32 w早产儿复筛听力障碍(86.67%)与胎龄34~36 w早产儿(50.00%)相比较高(P<0.05)。合并其他疾病早产儿复筛听力障碍(90.63%)与未合并其他疾病早产儿(60.00%)相比较高(P<0.05)。各组早产儿1年后随访听力障碍人数与复筛听力障碍人数比较,差异无统计学意义(P>0.05)。结论:自动听性脑干反应仪用于早产儿听力筛查中的临床作用显著,胎龄越小、合并其他疾病早产儿听力损伤发生危险性增高。Objective:To take the neonatal hearing screening for premature infants by automatic auditory brainstem response testing,and analyze the incidence of hearing impairment,in order to discover,diagnose and interfere the hearing disorders in early stage.Methods:250 cases of premature infants were randomly selected in our hospital,and the American ALGO3i automatic brainstem auditory evoked potential(AABR)was used for the hearing screening.30d after the screening,those who do not pass the hearing test were treated premature hearing test again.The results of screening,re-screening and follow-up after 1 year were compared.Results:The pass rate of premature infant initial hearing screening for gestational age 28~32W(66.67%)was significantly lower than that of gestational age 34~36W(87.50%)(P<0.05).The initial hearing screening rate of premature infants with other diseases(75.38%)was significantly lower than that of premature infants without other diseases(87.50%)(P<0.05).The hearing disorders rate of premature infant hearing rescreening for gestational age 28~32W(86.67%)was significantly higher than that of gestational age 34~36W(50.00%)(P<0.05).The hearing disorders rate of hearing rescreening in premature infants with other diseases(90.63%)was significantly higher than that of premature infants without other diseases(60.00%)(P<0.05).There was no significant difference in the number of hearing impairment between the rescreening and follow-up after 1 year(P>0.05).Conclusion:The automatic auditory brainstem response test has a significant effect on the hearing screening for premature infants.The results show that the smaller the gestational ages,the higher the risk of hearing damage,and preterm infants with other diseases have much higher risk of hearing damage.
