摘 要:目的探究中国西部偏远地区低收入维吾尔族人群中体质指数(body mass index,BMI)与肝功能指标的关系。方法分析了3 286名参与健康体检,具有完整个人信息和11项肝功能指标检测数据的维吾尔族成人。结果(1)在40至49岁之间,超重和肥胖者占总人数的比例达到高峰。(2)在体检维吾尔族人群中,超重和肥胖者分别占总人数的25.2%和8.8%。在11项生化指标中,7个与BMI呈正相关,分别是空腹血糖(blood glucose,GLU)、谷氨酰转肽酶(gamma-glutamyl transpeptidase,GGT)、门冬氨酸氨基转移酶(aspartate aminotransferase,AST)、丙氨酸氨基转移酶(alanine aminotransferase,ALT)、总胆红素(total bilirubin,TBIL)、直接胆红素(direct bilirubin,DBIL)和间接胆红素(indirect bilirubin,IBIL)。(3)与正常体重组相比,超重组的ALT和GGT的水平显著升高,而肥胖组的AST、ALT和GGT水平显著升高(均P<0.01)。(4)4组BMI分组之间,生化指标异常率有统计学差异的指标是AST。两组间比较发现,超重组及肥胖组较正常体重组的AST水平显著升高。(5)单因素Logistic回归分析发现,超重和肥胖是引起AST异常升高的危险因素。结论在新疆南部农村地区维吾尔族低收入人群中,超重与肥胖与肝功能指标异常升高相关,可能是AST异常升高的危险因素。Objective To investigate the relationship between body mass index(BMI)and liver function indices in low-income Uyghur population in remote areas of western China.Methods A total of 3286 Uyghur adults who participated in physical examinations and had complete personal information and 11 indices data of liver function were enrolled in this study.Results ①The highest proportions of overweight and obesity were observed in participants aged from 40 to 49 years.②The prevalence of overweight and obesity was as high as 25.2%and 8.8%,respectively,in this population.Among the 11 biochemical indicators of liver function,7 were positively correlated with BMI,namely blood glucose(GLU),gamma-glutamyl transpeptidase(GGT)and aspartate aminotransferase(AST),alanine aminotransferase(ALT),total bilirubin(TBIL),direct bilirubin(DBIL),and indirect bilirubin(IBIL).③The levels of ALT and GGT were much higher in overweight individuals than those in individuals with normal weight,and the levels of AST,ALT and GGT were significantly increased in obese individuals.④There was significant difference in AST levels among the 4 groups divided in terms of BMI.⑤The univariate Logistic regression analysis showed that obesity and overweight were risk factors for increased levels of AST.Conclusion In Uygur low-income populations in rural areas of southern Xinjiang,overweight and obesity are associated with abnormally elevated liver function,and may be risk factors for elevated AST.
