原发性干燥综合征关节炎肌骨超声半定量评分与疾病活动指数的相关性分析

摘　　要：目的:探讨原发性干燥综合征(pSS)关节炎肌骨超声半定量评分与疾病活动指数(SSDAI)的相关性。方法:选取2014年1月至2017年5月在本院住院治疗的pSS患者31例,根据超声诊断结果分为关节炎组18例,非关节炎组13例。应用高频超声对每个检查关节采用肌骨超声半定量评分法进行评分,根据SSDAI评分表对患者的整体病情进行评分,分析两者间的相关性。结果:关节炎组主要超声表现为滑膜炎,病变程度较轻,肌骨超声半定量评分项目以1~2分为主。关节炎组肌骨超声半定量评分为(13.0±8.52)分;非关节炎组0分。关节炎组SSDAI评分为(10.333±3.272)分,非关节炎组为(7.077±3.252)分,两组比较差异有统计学意义(t=2.741,P=0.01)。Pearson相关分析显示,关节炎组肌骨超声半定量评分与SSDAI评分呈正相关关系(r=0.793,P=0.000)。结论:pSS关节炎的肌骨超声半定量评分与整体疾病活动性关系密切。Objective:To investigate the correlation between the ultrasonic scoring system of arthritis and the assessment of Sjogren’s syndrome disease activity index(SSDAI)in patients with primary Sjogren’s syndrome(pSS).Methods:Thirty-one patients with pSS in our hospital from January 2014 to May 2017 were enrolled,and divided into arthritis group(n=18)and non-arthritis group(n=13)according to the results of ultrasound.Musculoskeletal ultrasound was performed and the images of the joints were scored using a semiquantitative scoring system.The correlation between the ultrasonic scoring system and the assessment of SSDAI was analyzed.Results:Most of the patients in arthritis group had light arthromeningitis by ultrasound,and the ultrasonic scores were mainly 1 to 2 with an average of 13.0±8.52.The non-arthritis group scored at 0.The SSDAI score was 10.333±3.272 in the arthritis group,and was 7.077±3.252 in non-arthritis group.There was significant difference between the two groups(t=2.741,P=0.01).Pearson correlation analysis revealed that musculoskeletal ultrasound semiquantitative scoring system was positively related to the SSDAI score(r=0.793,P=0.000).Conclusion:The ultrasonic scores for arthritis in pSS was closely correlated with the disease activity.
