胸腔镜同期双侧肺大疱切除手术治疗效果分析

摘　　要：目的:分析胸腔镜同期双侧肺大疱切除的手术治疗效果。方法:选取2012年1月-2015年1月收治的100例自发性气胸合并双侧肺大疱患者,分为两组,对照组患者行单侧胸腔镜肺大疱切除术治疗,观察组患者同期行胸腔镜双侧肺大疱切除术治疗,对比两组手术治疗效果。结果:两组住ICU时间、胸脏引流管留置时间、住院时间对比,差异无统计学意义(P>0.05);观察组引流量多于对照组,差异有统计学意义(P<0.05);观察组患者气胸发生率为8.00%,低于对照组的44.00%,对侧自发性气胸发生率为0,低于对照组的36.00%,差异均有统计学意义(P<0.05);两组术后漏气、呼吸衰竭、胸腔粘连、急性肺水肿等并发症发生率比较差异均无统计学意义(P>0.05)。结论:胸腔镜同期双侧肺大疱切除的手术治疗效果显著,可靠性与安全性高,能对对侧气胸的发生进行有效预防。Objective:To analyze the surgical treatment effect of thoracoscopic resection of bilateral bullae.Method:From January 2012 to January 2015,100 cases of spontaneous pneumothorax with bilateral bullae were divided into two groups,the control group was given unilateral thoracoscopic resection of lung bullae treatment,the observation group was given thoracoscopic bilateral pulmonary bullae resection,the effect of surgical treatment between the two groups were compared.Result:There was no significant difference between the two groups in ICU duration,chest drainage tube retention time and hospitalization time(P>0.05).The drainage volume of observation group was more than that of control group,the difference was statistically significant(P<0.05).The incidence of pneumothorax in the observation group was 8.00%,lower than 44.00%of the control group,the contralateral pneumothorax incidence was 0,lower than 36.00%in the control group,the differences were statistically significant(P<0.05).There was no significant difference in the incidence of postoperative air leakage,respiratory failure,thoracic adhesion and acute pulmonary edema between the two groups(P>0.05).Conclusion:The surgical treatment effect of thoracoscopic resection of bilateral bullae was of high reliability and safety,can occur in the contralateral pneumothorax.
