高龄骨质疏松性股骨粗隆间骨折行钢丝张力带固定加髋关节置换术的分析

摘　　要：目的研究高龄骨质疏松性股骨粗隆间骨折行钢丝张力带固定加髋关节置换术的临床应用效果。方法方便选取2013年2月—2015年2月于该院接受治疗的70岁以上的高龄骨质疏松性股骨粗隆间骨折患者92例进行研究,按照对他们治疗方式的不同把他们随机分为两组,其中观察组46例,我们对其进行钢丝张力带固定加髋关节置换术,对照组46例,对其进单纯的髋关节置换术,对比分析两组患者的治疗效果以及术后并发症发生情况。结果观察组患者在治疗总有效率95.72%、平均手术时间(52.65±9.33)min、切口长度(5.44±1.28)cm、术中出血量(127.65±7.59)mL、平均住院时间(17.65±5.12)d以及术后并发症发生概率17.48%均优于对照组,两组患者的对比差异有统计学意义(χ~2=8.425,t=20.126、8.924、37.689、15.237,χ~2=5.361,P﹤0.05)。结论在对高龄骨质疏松性股骨粗隆间骨折患者进行治疗的过程中,使用钢丝张力带固定加髋关节置换术对患者进行治疗能够取得更好的临床效果,显著缩短患者的手术时间以及住院时间,患者在手术中的伤口长度比较小,术中出血量比较少,患者在术后的并发症发生概率也比较低,临床治疗效果显著,值得在临床上进行广泛的推广与应用。Objective This paper tries to study the clinical effect of tension band fixation and hip replacement for the osteoporotic intertrochanteric fractures of the elderly.Methods Convenient selected 92 cases of patients with methods from February,2013 to January 2015 year in February in our hospital over the age of 70 elderly patients with osteoporotic intertrochanteric fractures were investigated.According to their different treatment methods they were randomly divided into two groups:the observation group of 46 case with wire tension band fixation of hip arthroplasty while the control group of 46 case with the total hip arthroplasty in the comparative analysis of two groups of patients and postoperative complications.Results The overall response rate in treatment of the patients in the observation group was 95.72%,with the average operation time(52.65±9.33)min,the length of the incision(5.44±1.28)cm,the amount of bleeding during operation(127.65±7.59)mL,the average hospitalization time(17.65±5.12)d and postoperative complication probability of 17.48%better than the control group.There was statistical significance comparison between the two groups(χ2=8.425,t=20.126,8.924,37.689,15.237,χ2=5.361,P<0.05).Conclusion For elderly osteoporotic intertrochanteric fractures treated,using steel wire tension can achieve better clinical effect in the treatment of patients with fixation and hip replacement,shorten the operation time and hospitalization time of patients.The wound length of patients in the surgical is relatively small,the amount of intraoperative bleeding is fewer and the occurance probability of patients in the postoperative complication is relatively low,and the clinical efficacy is significant,which is worthy of promotion and application widely in clinic.
