摘 要:目的探讨CT与MRI增强扫描应用于原发性胰腺癌临床诊断中的价值差异,为临床应用提供参考依据。方法回顾性分析该院2015年1月—2017年1月所收治40例胰腺癌(均经病理检查确诊)患者的CT、MRI增强扫描等影像学资料。比较其特征性成像表现差异。结果 CT增强扫描确诊率为83.33%,与病理检查的100.0%相比较,前者确诊率稍低,差异无统计学意义(χ~2=2.455,P>0.05)。MRI增强扫描确诊率为87.5%,与病理检查的100.00%相比较,前者确诊率稍低,差异无统计学意义(χ~2=0.533,P>0.05)。CT检查示>1.0 cm病灶25个,<1 cm病灶3个,而病理检查结果示>1 cm病灶27个,<1 cm病灶1个,实际错误率为3.57%;MRI检查示>1.0 cm病灶22个,<1 cm病灶2个,而病理检查结果示>1 cm病灶23个,<1cm病灶1个,实际错误率为4.17%;2种检查实际错误率相比,差异无统计学意义(χ~2=0.019,P>0.05)。结论 CT与MRI增强扫描应用于原发性胰腺癌临床诊断中,均具有较好的诊断价值,可靠性高;其中,MRI显示细节能力更强,CT检查更为便捷,二者应灵活运用,以提升临床诊疗水平。Objective This paper tries to analyze to explore the value difference between CT and MRI enhanced scan in the clinical diagnosis of primary pancreatic cancer,and to provide references for clinical application.Methods The imaging data of 40 cases of pancreatic cancer(all confirmed by pathological examination)in our hospital from the January of 2015 to the January of 2017 were analyzed retrospectively,such as CT,MRI enhanced scan and so on.The differences in characteristic imaging were compared.Results The diagnostic accuracy of CT enhanced scan was 83.33%.Compared with 100.00%of pathological examination,the difference was not statistically significant(χ2=2.455,P>0.05).The diagnostic accuracy of MRI enhanced scan was 87.5%.Compared with 100.00%of pathological examination,but the difference was not statistically significant(χ2=0.533,P>0.05).CT more than 1.0 cm showed 25 lesions,and less than 1cm showed 3 lesions,and the pathological examination showed 27 lesions more than 1cm,and 1 lesion less than 1cm.The actual error rate was 3.57%;MRI more than 1.0 cm showed 22 lesions,2 lesions less than 1cm,and the pathological examination showed 23 lesions,and less than 1cm 1 lesion.The actual error rate was 4.17%;compared with the two methods of checking actual error rates,the difference was no statistically(χ2=0.019,P>0.05).Conclusion CT and MRI enhanced scan are applied in the diagnosis of primary pancreatic cancer.They all have good diagnostic value and high reliability.Among them,MRI shows more detail ability and CT examination is more convenient.The two should be flexible in order to improve the level of clinical diagnosis and treatment.
