影响胃十二指肠溃疡穿孔修补术后再穿孔的相关因素分析

摘　　要：目的观察分析影响胃十二指肠溃疡穿孔修补术后再穿孔的相关因素。方法回顾性分析该院2012年5月—2017年5月收治的76例进行穿孔修补术,其中有9例患者术后再穿孔,分析影响胃十二指肠溃疡穿孔修补术后再穿孔的相关因素。结果胃十二指肠溃疡穿孔修补术后再穿孔患者有6例患者术式选择不当,有7例患者手术操作不当,有6例患者术前休克,有6例患者合并慢性疾病,有8例患者首次穿孔或溃疡面超过1.2 cm,有2例为癌性穿孔,9例胃十二指肠溃疡穿孔修补术后再穿孔患者其中有3例患者经胃肠减压保守治疗,有5例经胃大部分切除术治疗,有1例行穿孔后再次修补,所有患者均治愈出院。结论对于胃十二指肠溃疡穿孔患者应选择合适的治疗方式,严格按照无菌操作的要求进行,从而预防胃十二指肠溃疡穿孔修补术后再穿孔发生。Objective This paper tries to observe and analyze the related factors of perforation after repair of perforated gastroduodenal ulcer.Methods A retrospective analysis from May 2012 to May 2017 of 76 cases were perforation repair,including 9 cases of patients with postoperative perforation,analysis of factors related to surgical repair of gastroduodenal ulcer perforation after perforation.Results After surgical repair of gastroduodenal ulcer perforation and perforation in patients with 6 cases of patients with type 7 patients with improper selection,improper operation,6 cases of patients with preoperative shock,6 cases of patients with chronic diseases,8 cases of patients with ulcer perforation or for the first time more than 1.2 cm,there were 4 cases of gastric cancer perforation,9 cases of gastroduodenal ulcer perforation after perforation patients including 3 cases of patients with gastrointestinal decompression after conservative treatment,5 patients after subtotal gastrectomy treatment,1 cases of perforation again after repair,all patients were cured.Conclusion For patients with gastroduodenal ulcer perforation,appropriate treatment should be selected,and strictly according to the requirements of aseptic operation,so as to prevent the occurrence of re perforation after repair of gastroduodenal ulcer perforation.
