摘  要:目的探讨妊娠期糖尿病患者应用胰岛素泵短期强化干预、生物合成人胰岛素联合治疗对其血糖控制效果及母婴结局的影响情况。方法利用随机数字表法方便选取2016年4月—2017年2月期间该院收治的90例妊娠期糖尿病患者,分为研究组、对照组(每组45例)。对照组在基础治疗期间加用胰岛素泵短期强化干预,研究组在基础治疗期间加用胰岛素泵短期强化干预、生物合成人胰岛素。记录两组治疗前后血糖(FBG、2 h PBG)检测结果变化情况、低血糖发生率及妊娠结局。结果两组治疗前血糖指标检测值对比差异无统计学意义(P>0.05),其中研究组治疗前FBG为(6.55±1.34)mmol/L、2 h PBG为(8.59±1.37)mmol/L,对照组治疗前FBG为(6.57±1.40)mmol/L、2 h PBG为(8.64±1.35)mmol/L;治疗后研究组FBG、2 h PBG下降幅度优于对照组(P<0.05),其中研究组治疗后FBG为(4.93±0.52)mmol/L、2h PBG为(6.11±0.98)mmol/L,对照组治疗后FBG为(5.68±0.79)mmol/L、2 h PBG为(7.65±1.13)mmol/L。研究组剖宫产率仅为31.11%(14/45)、对照组高达66.67%(30/45)(P<0.05);除两组患者夜间低血糖发生率对比差异无统计学意义(P>0.05),分别为8.89%、4.44%;研究组患者、新生儿各项不良反应发生情况均显著低于对照组(P<0.05)。结论胰岛素泵短期强化干预、生物合成人胰岛素联合治疗妊娠期糖尿病可显著提高患者血糖控制效果,有利于其获得更为理想的母婴结局。Objective This paper tries to investigate the effects of short-term intensive intervention with insulin pump and biosynthesis of human insulin on blood glucose control and maternal and neonatal outcomes in gestational diabetes mellitus.Methods 90 cases of gestational diabetes mellitus treated in our hospital from April 2016 to February 2017 were convenient selected and randomly divided into the study group and the control group(with 45 cases in each group).In the control group,the short-term intensive intervention was added to the insulin pump during the basic treatment.During the basic treatment period,the study group was treated with short-term intensive insulin pump intervention and biosynthesis of human insulin.The changes of blood glucose(FBG,2 hPBG)test results,the incidence of hypoglycemia and the outcome of pregnancy were recorded before and after treatment in two groups.Results The blood glucose index contrast between the two groups before treatment(P<0.05),in the study group before treatment,FBG(6.55±1.34)mmol/L,2 hPBG(8.59±1.37)mmol/L,the control group before treatment,FBG(6.57±1.40)mmol/L,2 hPBG(8.64±1.35)mmol/L;after treatment in the study group,FBG 2 hPBG decreased more than the control group(P<0.05),one of the study group after treatment for FBG(4.93±0.52)mmol/L,2 hPBG(6.11±0.98)mmol/L,the control group after treatment for FBG(5.68±0.79)mmol/L,2 hPBG(7.65±1.13)mmol/L.Study on the cesarean section rate was only 31.11%(14/45),the control group reached 66.67%(30/45)(P<0.05);the two group patients with nocturnal hypoglycemia was compared with no significant difference(P>0.05),respectively 8.89%and 4.44%of patients in the study group,the newborn;the incidence of adverse reactions were significantly lower than the control group,comparison of the data(P<0.05).Conclusion Short term intensive insulin pump intervention and biosynthesis of human insulin in the treatment of gestational diabetes mellitus can significantly improve the blood glucose control effect of patients,and help them get better mate
