高龄患者腹腔镜胃癌根治术不同麻醉方法的比较

摘　　要：目的:比较高龄胃癌患者腹腔镜根治术应用瑞芬太尼和舒芬太尼的复合麻醉效果。方法:收治≥70岁老年胃癌根治术患者78例,分两组。R组实施瑞芬太尼静脉麻醉,S组实施舒芬太尼静脉麻醉,比较苏醒时间、定力恢复时间、术后24 h疼痛等,记录不同阶段心率HR、平均动脉压MAP值。结果:R组苏醒时间、定力恢复时间等短于S组,术后24 h疼痛较R组重(P<0.05)。结论:舒芬太尼和瑞芬太尼均能取得良好的麻醉效果,瑞芬太尼起效快、恢复快,对血流动力学影响小,更加安全、有效,舒芬太尼术后镇痛效果好。Objective:The effect of combined anesthesia with remifentanil and sufentanil in laparoscopic radical mastectomy for elderly patients with gastric cancer.Methods:78 cases of elderly patients over 70 years old who were treated with radical gastrectomy were selected.They were divided into the two groups.Intravenous anesthesia with remifentanil was applied in the group R.The group S was administered sufentanil for intravenous anesthesia.The time of awakening,the recovery time of the force and the 24 h pain after operation were recorded,and the heart rate HR and the mean arterial pressure MAP at different stages were recorded.Results:The time of awakening,the recovery time of the force in the R group was shorter than that of the S group,and the 24 h pain after the operation was more severe than that in the group R(P<0.05).Conclusion:Both sufentanil and remifentanil can achieve good anesthetic effect,remifentanil has fast effect,fast recovery,little influence on hemodynamics,and more safe and effective.Sufentanil can get good analgesic effect after operation.
