精神障碍诊断软件在精神科诊断质量上的管理应用研究

摘　　要：目的:研究《社区精神患者诊断与公共安全风险评估软件》在精神科诊断质量控制中的临床意义。方法:收治精神障碍患者200例,通过主管医师常规精神检查得出明确人工诊断,然后以计算机软件测试当事人和知情人,得到软件诊断。结果:200例患者中,临床医生诊断精神障碍阳性率188例(94.42%),软件检查当事人版本182例(92.39%),知情人版本190例(96.45%),两软件综合计算诊断193例(97.97%),3者间差异无统计学意义(P>0.05)。知情人对人格问题的描述高于工作检查方法,差异有统计学意义(P<0.05)。不明确诊断合计,软件当事人版本8例(4.06%),医生临床未明确诊断11例(5.58%),组间差异无统计学意义(P>0.05)。软件综合检查未能明确诊断4例(2.03%),与医生人工诊断差异有统计学意义(P<0.05)。结论:《社区精神患者诊断与公共安全风险评估》软件在临床中,总体与人工诊断效果接近,可以作为医院管理过程中核实临床诊断水平的参考工具。Objective:To study the clinical significance of"community mental patient diagnosis and public safety risk assessment software"in the quality control of psychiatric diagnosis.Methods:200 patients with mental disorders were selected.On the one hand,through the routine psychiatric examination of the chief physician,it is clear that the artificial diagnosis is made.Then the computer software is used to test the client and the insiders to make the software diagnosis,and get the software diagnosis.Results:In 200 patients,the positive rate of clinicians diagnosing mental disorders was 188 cases(94.42%),182 cases(92.39%)of litigant version by software,190 cases(96.45%)with the insider version,two software comprehensive diagnosis of 193 cases(97.97%),and there was no statistically significant difference in the 3 groups(P>0.05).The description of the person's personality problem was higher than the examination method,and the difference was statistically significant(P<0.05).No clear diagnosis,software parties version in 8 cases(4.06%),there was no statistical difference between 11 cases(5.58%)of undiagnosed clinical diagnosis with a doctor(P>0.05).Failure to diagnose in 4 cases by software comprehensive examination,and there was a significant difference between manual diagnosis by doctors(P<0.05).Conclusion:The software of"community mental patient diagnosis and public safety risk assessment"is close to the artificial diagnosis in clinical.It can be used as a reference tool to verify the level of clinical diagnosis in the process of hospital management.
