摘　　要：目的:探讨连续护理模式在社区老年慢性病患者护理中的应用效果。方法:收治老年慢性病患者150例,随机分为观察组和对照组。对照组采用常规护理,观察组在对照组的基础上加用连续护理模式。结果:观察组患者慢性病知识掌握情况、基本护理技能评分、SAS评分、SDS评分、护理满意度均明显优于对照组(P<0.05)。结论:连续护理模式在社区老年慢性病患者护理中的应用效果显著。Objective:To explore the application effect of continuous nursing model in the nursing of chronic diseases of the elderly in the community.Methods:150 elderly patients with chronic diseases were chosen,and they were randomly divided into the observation group and the control group.The control group was given routine nursing,and the observation group added with the continuous nursing mode on the basis of the control group.Results:In the observation group,patients'knowledge of chronic diseases,basic nursing skills,SAS score,SDS score and nursing satisfaction were significantly better than those in the control group(P<0.05).Conclusion:The application effect of continuous nursing model in the nursing of chronic diseases of the elderly in the community was significant.
