益气养心安神汤治疗慢性阻塞性肺疾病稳定期患者抑郁状态临床研究

摘　　要：目的观察益气养心安神汤治疗慢性阻塞性肺疾病(COPD)稳定期患者抑郁状态的临床疗效。方法采用随机平行对照法将138例COPD稳定期合并抑郁患者分为对照组和研究组各69例。对照组予氧疗、肺康复治疗,沙美特罗替卡松粉吸入剂,每次1吸,每日2次。研究组在对照组基础上予益气养心安神汤,每日1剂,早晚2次口服。均连续治疗1个月。观察2组治疗前后中医症状评分、汉密顿抑郁量表(HAMD)、医院焦虑抑郁情绪测量表(HAD)评分,记录治疗前后第一秒用力呼气容积(FEV1)、第一秒用力呼气容积占用力肺活量百分比(FEV1/FVC)。结果与本组治疗前比较,2组治疗后中医症状评分明显降低,研究组治疗后HAMD、HAD评分明显降低(P<0.05);研究组中医症状评分、HAMD、HAD评分明显低于对照组(P<0.05)。与本组治疗前比较,2组治疗后肺功能(FEV1、FEV1/FVC)明显改善(P<0.05),研究组肺功能改善明显优于对照组(P<0.05)。结论益气养心安神汤在常规西药治疗基础上,可明显改善COPD稳定期患者抑郁状态及肺功能,提高生活质量。Objective To observe the efficacy of Yiqi Yangxin Anshen Decoction in the treatment of depression in patients with stable chronic obstructive pulmonary disease(COPD).Methods Totally 138 COPD patients were divided into two groups through randomized parallel control method,with 69 cases in each group.The control group was given oxygen therapy,pulmonary rehabilitation,salmeterol fluticasone powder inhaler,one dose per time,twice a day.On the basis of control group,the treatment group was given Yiqi Yangxin Anshen Decoction,one dose per day,one time for morning and one time for evening,orally.The treatment course was one month.TCM syndrome quantification score,HAMD score and HAD score in both groups before and after treatment were observed.The changes of FEV1 and FEV1/FVC before and after treatment were recorded.Results Compared with before treatment,TCM syndrome quantitative scores,HAMD scores and HAD scores in both groups were significantly reduced(P<0.05),and the treatment group decreased more significantly,with statistical significance(P<0.05).Compared with before treatment,FEV1 and FEV1/FVC in both groups were improved significantly(P<0.05),and the treatment group was better than the control group(P<0.05).Conclusions Yiqi Yangxin Anshen Decoction combined with routine Western medicine for the treatment of depression can improve the depression condition of stable COPD patients,improve pulmonary functions and life quality.
