强筋壮骨丸辅助经皮椎体后凸成形术治疗骨质疏松性椎体压缩骨折临床研究

摘　　要：目的观察强筋壮骨丸辅助经皮椎体后凸成形术(PKP)治疗骨质疏松性椎体压缩骨折(OVCF)的临床疗效。方法采用随机数字表法将68例OVCF患者分为对照组和治疗组各34例。2组均行PKP,术后予碳酸钙/维生素D3片,每日600 mg,口服,不少于6个月。治疗组在上述治疗基础上予强筋壮骨丸,每次6 g,每日3次,口服,连续治疗12个月。观察2组治疗前及术后1、6、12个月的疼痛视觉模拟评分(VAS)、Oswestry功能障碍指数评分(ODI)、伤椎椎体前缘高度、腰椎骨密度(BMD)及并发症。结果与本组术前比较,2组术后1、6、12个月VAS、ODI、伤椎前缘高度均明显改善(P<0.05);2组术后12个月腰椎BMD高于术前及术后1、6个月(P<0.05,P<0.01);治疗组术后6、12个月VAS、ODI、伤椎前缘高度、腰椎BMD优于对照组(P<0.05)。2组术中、术后均未发生神经损伤,治疗组、对照组骨水泥渗漏分别5、4例(P>0.05)。结论强筋壮骨丸辅助PKP治疗OVCF可改善患者疼痛,提高生活质量,增加椎体BMD。Objective To study the clinical efficacy of Qiangjin Zhuanggu Pills combined with percutaneous kyphoplasty(PKP)therapy on osteoporotic vertebral compression fracture(OVCF).Methods Totally 68 OVCF patients were randomly divided into control group and treatment group,with 34 cases in each group.Both groups were treated by PKP and received oral vitamin D3 and caltrate daily,while those in the treatment group were given Qiangjin Zhuanggu Pills additionally,once 6 g,three times a day,orally,for twelve months.Before and in 1,6,and 12 months after the surgery,visual analogue scale(VAS),Oswestry disability index(ODI),the height of the anterior border of vertebral bod,BMD of the lumbar spine and complications in the two groups were observed and compared.Results Compared with before the treatment,patients in the two groups showed significant improvements in the VAS,ODI and vertebral height 1,6,and 12 months after the surgery(P<0.05);During 6,and 12 months after the treatment,compared with the control group,the treatment group showed notable increases in the VAS,ODI,vertebral height and BMD of the lumbar spine(P<0.05).Neither nerve injury occurred in the two groups during surgery or postoperatively,but with 5 and 4 cases of cement leakage in the treatment group and the control group(P<0.05).Conclusion Qiangjin Zhuanggu Pills assisted PKP can increase BMD,reduce the pain,improve the quality of life and increase the BMD in OVCF patients.
