摘  要:目的观察不同电针刺激强度对糖尿病胃轻瘫(DGP)大鼠胃肠运动和饥饿素的影响,探讨不同电针刺激强度治疗DGP的效应差异。方法 60只SD大鼠随机分为空白组、模型组和小、中、大刺激量组,每组12只。采用单次腹腔注射2%链脲佐菌素配合高脂高糖饮食建立DGP大鼠模型。造模成功后,分别采用电针小、中、大刺激量进行干预,连续15 d。用血糖仪和血糖试纸每周测定血糖;治疗结束后以酚红为标记物,观察大鼠胃排空率及小肠推进率;ELISA检测血清饥饿素含量,RT-PCR检测下丘脑饥饿素的mRNA表达,免疫组化检测下丘脑饥饿素的蛋白表达。结果与空白组比较,模型组大鼠胃排空率及小肠推进率显著降低,血清饥饿素含量、下丘脑饥饿素mRNA表达显著降低,下丘脑饥饿素蛋白表达显著升高;与模型组比较,各针刺组大鼠胃排空率、小肠推进率、血清饥饿素含量、下丘脑饥饿素mRNA表达显著升高,下丘脑饥饿素蛋白表达明显降低,且大刺激量组优于中、小刺激量组。结论电针可通过上调饥饿素含量有效促进DGP大鼠胃肠运动,改善胃排空迟缓症状,且大刺激量组疗效更佳。Objective To observe the effects of different electroacupuncture(EA)intensity on gastrointestinal motility and expression of Ghrelin in hypothalamus in diabetic gastroparesis(DGP)rats;To explore the effedt differences of different EA intensity.Methods Sixty rats were randomly divided into normal control group,model group,weak EA stimulation group,middle EA stimulation group,and strong EA stimulation group,with 12 rats in each group.The DGP model was established by intraperitoneal injection of 2%streptozotocin and raised by high-sugar and high-fat fodder.After modeling,each EA stimulation group was intervened by EA with different intensity,for 15 d.Blood glucose was measured weekly with blood glucose meter and blood glucose test strips.After the treatment,phenol red was taken as a marker to observe the gastric emptying rate and small intestine propulsion rate.Serum Ghrelin levels were detected by ELISA,and mRNA expression of Ghrelin in the hypothalamus was detected by RT-PCR.Immunohistochemical detection was used to detect hypothalamic Ghrelin protein expression.Results Compared with the normal control group,gastric emptying rate and small intestine propulsion rate of the model group decreased;the content of Ghrelin in the serum.and expression of Ghrelin mRNA of hypothalamus also decreased significantly;gray value of Ghrelin of hypothalamus increased significantly.Compared with the model group,the gastric emptying rate,small intestine propulsion rate,content of Ghrelin in the serum,and the expression of Ghrelin mRNA of hypothalamus significantly increased in the three EA stimulationgroups,and the gray value of the hypothalamic Ghrelin significantly decreased.The effect of strong EA stimulation group was better than weak EA stimulation group and middle EA stimulation group.Conclusion EA can effectively promote bowel movement,and improve the symptoms of delayed gastric emptying of DGP rats by increasing Ghrelin content.The curative effect of strong EA stimulation group is better.
