咳与喘不同主症肺纤维化患者证候规律研究

摘　　要：目的:探讨咳组与喘组肺纤维化患者的中医临床证候规律。方法:采用调查问卷的方法,收集咳组与喘组患者的四诊信息,比较两组患者的病位和病性证候要素特点。结果:两组病位主要在肺脾肾,咳组病位多累及于肝,喘组病位多累及于心。在病性证素分布中,两组虚实分布不同。在虚性证素中,两组都以气虚为主,其次是阴虚、阳虚。在实性证素中,咳组兼夹实证较多,以火热、痰湿、血瘀多见,喘组实证以兼夹血瘀多见。结论:肺纤维化总的病机特点是本虚标实,本虚以气虚为主,标实以血瘀、火热、痰湿为主。咳组的病机特点与肺痹类似,喘组的病机特点与肺痿类似。Objective:To investigate the clinical syndromes of traditional Chinese medicine in patients with pulmonary fibrosis in cough and asthma group.Methods:Using the questionnaire to collect the four diagnostic information of the patients in the cough group and the asthma group,and compare the characteristics of syndrome elements of disease location and of disease nature between the two groups.Results:The distribution of disease location in the two groups were mainly in the lung,spleen and kidney.Cough group was more involved in the liver,asthma group was more involved in the heart.In the distribution of syndrome elements of disease nature,the two group was different.Deficiency syndrome elements of disease nature,both groups were mainly qi deficiency,followed by yin deficiency,yang deficiency.In the distribution of excesssyndrome elements of disease nature,the cough group combined more excess-syndrome,fire,phlegm,blood stasis more common.In the asthma group,blood stasis was more common.Conclusion:The total pathogenesis of pulmonary fibrosis is characterized by the deficiency in origin and excess in superficiality.The deficiency in origin is main in qi deficiency,and excess in superficiality is main in blood stasis,hot and phlegm.The pathogenesis of the cough group is similar to that of the lung paralysis.The pathogenesis of the asthma group is similar to the lung impotence.
