血清钙结合蛋白S100A_(12)及可溶性sRAGE在急性胰腺炎患者血清中的表达及意义

摘　　要：目的:探讨急性胰腺炎(AP)患者血清钙结合蛋白S100A_(12)及可溶性糖基化终末产物受体(sRAGE)蛋白的表达及临床意义。方法:将110例急性胰腺炎(AP)患者分为轻症组(MAP 54例)和重症组(SAP 56例),并以45例健康献血者作为对照组。入院后获取APACHEII评分,采用双抗体夹心酶联免疫吸附法测定患者外周血中S100A_(12)、sRAGE在发病48h内的水平,使用受试者工作特征曲线(ROC)下面积分析S100A_(12)、sRAGE、PCT、APACHEII评分在区分重度和轻度急性胰腺炎方面的价值。结果:SAP组S100A_(12)、sRAGE、PCT、APACHEII评分的水平显著高于对照组、MAP组,差异有统计学意义(P<0.05)。MAP组和对照组各项指标之间比较差异无统计学意义(P>0.05),SAP组死亡者S100A_(12)、sRAGE水平明显高于存活者(P<0.05)。ROC曲线显示,S100A_(12)对SAP组的预测价值相关指标:敏感度81.42%、特异度93.28%,曲线下面积0.86,sRAGE对SAP的预测价值相关指标:敏感度66.54%、特异度92.38%,曲线下面积0.81,联合检测S100A_(12)和sRAGE的敏感度63.74%、特异度95.73%。结论:S100A_(12)、sRAGE与AP的严重程度和预后密切相关,S100A_(12)、sRAGE有可能成为重症急性胰腺炎早期诊断的新指标。Objective:To study the expression and significance of S100A 12 and sRAGE in patients with acute pancreatitis.Methods:110 patients with acute pancreatitis were divided into severe acute pancreatitis(SAP)group and moderate acute pancreatitis(MAP)group,45 healthy blood donors as control group.The APACHEII scores were collected after admission,serum level of S100A 12,sRAGE were detected by enzymelinked immunosorbent assay(ELISA)within 48 hours.The area under the curve(AUC)of the receiver operating characteristic curve(ROC)of S100A 12,sRAGE,PCT levels and APACHEII scores was used to estimate the severity of AP.Results:Serum levels of S100A 12、sRAGE、PCT and APACHEII scores were significantly higher in the SAP group than MAP and the control group(P<0.05).There were no difference on them between the MAP group and control group(P>0.05).S100A 12 and sRAGE of the death patients in SAP group were higher than those of the survivors(P<0.05).The sensitivity,specificity and AUC of S100A 12 in diagnosing SAP were 81.42%,93.28%and 0.86,while the sensitivity,specificity and AUC of sRAGE were 66.54%,92.38%and 0.81.The sensitivity,speoailty of the combination use of S100A 12 and sRAGE were 63.74%and 95.23%.Conclusion:The levels of S100A 12 and sRAGE are associated with the severity and prognosis of AP,and it may be useful for the early diagnosis of SAP.
