神经内镜联合显微镜微血管减压术在三叉神经痛及面肌痉挛治疗中的应用

摘　　要：目的探讨神经内镜联合显微镜微血管减压术在三叉神经痛及面肌痉挛治疗中的效果。方法回顾性分析2012年1月~2016年12月期间采用神经内镜联合显微镜进行微血管减压术治疗的35例三叉神经痛及25例面肌痉挛患者的临床资料,术后随访,评估分析手术治疗效果、术后并发症及复发情况。结果60例患者术中视野显露效果均满意,共发现责任血管58例,其中三叉神经痛组33例,面肌痉挛组25例。三叉神经痛组治疗有效率94%,随访期间复发2例,复发率6%。面肌痉挛组治疗有效率100%,无复发。两组之间有效率无差异(P>0.05)。结论神经内镜联合显微镜微血管减压术治疗三叉神经痛及面肌痉挛,提高了微血管减压术的疗效和安全性,具有较好的应用前景。Objective To evaluate the clinical effect of endoscope and microvascular decompression in the treatment of trigeminal neuralgia(TN)and hemifacial spasm(HFS).Methods A total of 60 patients including 35 cases of TN and 25 cases of HFS undergone microvascular decompression surgery via endoscope combined with microscope from January 2012 to December 2016 was reviewed respectively.The operation efficiency,postoperative recurrence and complications were evaluated according to subjective and objective criteria.Results The intraoperative visual field exposure were satisfactory in 60 patients.58 cases of all had responsible vessels,33 cases were in trigeminal neuralgia group,mainly superior cerebellar artery,while 25 cases were in hemifacial spasm group,mainly anterior inferior cerebellar artery.The operation efficiency was 94%and 2(6%)cases recurred during follow-up in TN group.The operation efficiency was 100%and no one recurred in HFS group.there was no difference in the operation efficiency and the rate of complication between the two groups(P>0.05).Conclusion A combination use of endoscope and microscope could provide better exposure with minimal invasive,the efficacy and safety of microvascular decompression surgery especially for treating TN and HFS.
