摘 要:目的:探讨左旋甲状腺素钠(LT4)治疗先天性甲状腺功能低下症患儿的临床效果及对远期预后的影响。方法:选取100例诊断为先天性甲状腺功能低下症患儿作为LT4组,采用LT4行替代治疗,对比患儿治疗前后的甲状腺功能激素水平,随访2年后的生长发育及智能发育情况并与选取的100例健康儿童(健康组)进行对比。结果:治疗1个月后,LT4组的血清促甲状腺激素较治疗前显著降低(P<0.05),LT4组的FT3、FT4水平较治疗前显著升高(P<0.05);出生时LT4组和健康组的身长、体质量、头围差异无统计学意义(P>0.05);出生6个月后LT4组的身长、体质量、头围与健康组差异均无统计学意义(P>0.05);2岁时LT4组患儿的C-WYCSI量表的语言智商(VIQ)、操作智商(PIQ)、总智商(FIQ)评分与健康组比较差异均无统计学意义(P>0.05)。结论:先天性甲状腺功能低下症患儿采用LT4替代治疗能迅速纠正患儿甲低状况,促进患儿身体及智力发育。Objective:To investigate the clinical effect of levothyroxine sodium(LT4)in the treatment of children with congenital hypothyroidism and its effect on long-term prognosis.Methods:100 children with congenital hypothyroidism were selected as LT4 group,LT4 replacement therapy was used,the levels of thyroid hormones in children before and after treatment were compared,and the growth development and mental development were observed after 2 years of follow-up.The results were compared with those of 100 healthy children(healthy group).Results:After 1 month of treatment,the serum thyroid stimulating hormone decreased significantly in LT4 group than that before treatment(P<0.05),FT3 and FT4 levels in LT4 group significantly increased than those before treatment(P<0.05);The connatal body length,body weight and head circumference differences were not statistically significant in LT4 group and healthy group(P>0.05);After 6 months of age,the body length,body weight,head circumference had no significant differences between two groups(P>0.05);At the age of 2,C-WYCSI scale VIQ,PIQ and FIQ had no significat differences between two groups(P>0.05).Conclusion:LT4 replacement therapy can quickly correct the situation,promote physical and intelligence development for children with congenital hypothyroidism.
